NRC-10-87-443 Modification No. 12 Page 2 of 3 The purpose of this modification is to: (1) extend the period of performance ,
through 3/26/98; (2) revise the Statement of Work to incorporate an option I year, specify supply requirements, identify Government furnished property, and revise the reporting requirements, as attached; (3) provide incremental funding in the amount of $1,861.03 which increases the cooperative agreement !
ceiling and total obligated amounts from $16,791.52 to $18,652.55 and (4) {
change the Cooperator name and address. Accordingly, the following changes are hereby made to the " Notice of Cooperative Agreement":
