Gentlemen:
The subject inspection report identified three (3) infractions and one (1) deviation from comnitment to the Commission.
Our response to these items is provided below:
Item 1:
Contrary to the requirements of T.S. 3.5.3 for Mode 3 operation, 3 HPSI pump
. handswitches were found to be in the " Pull-to-Lock" position.
Two of the HPSI pump handswitches were immediately returned to the " Normal-After-Stop" status to conform.to the Technical Specifications requirements.
The-failure to return the handswitches to normal was a result of inadvertent omission of that step in the controlling procedure (OP 2102.03) in use at that time.
That procedure has since been revised to include sign-off steps to return the HPSI pump handswitches to normal.
Plant controlling procedures 2102.02, 2102.03, and 2102.10 were reviewed in their entirety for similar errors.
Some unrelated adjustments were made based on these reviews.
We feel we are in compliance with Technical Specifications, and this is not a recurrent problem.
