NOTICE OF VIOLATION Hardin Memorial Hospital License No. 34-16032-01 Based on the inspection conducted-on September 22, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
Item 1 is an infraction and item 2 is a deficiency.
1.
Condition 18 of your license requires that licensed material be poss(ssed and used in accordance with statements, repre-sentations, and procedures contained in application dated February 19, 1974 and letter dated May 24, 1977.
Item 7 of supplemento./ data attached to your application and entitled "Mo99/Tc99m Geuerators" requires that eluted material be assayed radiometrically for Tc99m to a minimum of 90%
accuracy.
Contrary to this requirement, on May 17, 1978, you failed to assure the accuracy of your dose calibrator when a radioassay of Tc99m standard indicated a dose of 16% in excess of the stated activity.
2.
10 CFR 20.401(b) requires that you maintain records showing the results of surveys required by 10 CFR 20,201(b) " Surveys."
Contrary to this requirement, as of September 22, 1978, you f ailed to maintain records of surveys that you performed to assure compliance with 20.301, " Waste Disposal, General Requ1rements," a regulation that describes authorized means of disposing of licensed material contained in vaste.
