COMANCHE PEAK STEAM ELECTRIC STATION (CPSES)

DOCKET N05, 50-445 AND 50-446 AIR ACCUMULATORS FOR CONTROL VALVES SDAR: CP-87-15 (FINAL REPORT)

Gentlemen:

On May 20, 1987, we verbally notified your Mr. 1. Barnes of a deficiency involving the air accumulators for air-operated control valves. After further evaluation we have detennined that this deficiency is reportable under the provisions of 10CFR50.55(e). Our last interim report was logged Txx-6962, dated November 13, 1987.

DESCRIPTION OF DEFICIENCY The deficiency involves the absence of installation records for QC inspection of the location, erection, tolerance cnecks and the installation of these accumulators.

Also, upon inspection, desiccant was found in twelve (12) accumulators and rust / scale formation was noticed in six (6) accumulators (CPI-CIATAF-01,02,03,04,07,08) after installation was completed.

Desiccant is provided to inhibit rust formation during transit and storage of these accumulators.

The accumulators are made of carbon steel and the associated instrument tubing is of stainless steel.

A total of twenty-two (22) air accumulators, for Units 1 and 2, which serve various safety-related systems, are affected by this deficiency. Of these, eighteen (18) are required to perform a safety function.

o TXX-88009 January 18, 1988 Page 2 of 2 The other four (4) accumulators (CP1-CIATHS-01,02 and CP2-CIATMS-01,02) are in the main steam system, but the associated valves assume their fail-safe positions upon loss of air, so system function would not be impaired.

The cause of the hardware deficiencies was failure to remove the desiccant from the accumulators prior to installation.

This issue is limited to the associated accumulators.

The desiccant has been removed from the affected accumulators and the accumulators have been cleaned.

The rust formations have been attributed to humidity in the ambient air.

This will be corrected when the accumulator systems can be charged with dry air, per design.

The lack of installation records for the a'.cumulators is due to originally misidentifying the accumulators as instrumentation items instead of mechanical items.

SAFETY IMPLICATIONS Had this condition remained uncorrected for the twelve (12) accumulators which had not han the desiccant removed, operation of the associated safety-related systems could have been adversely affected.

CORRECTIVE ACTIONS Nonconformance reports NCR CI-87-5103-X R1, 186-103786 R-1, IMP-87-007 and test deficiencies report TOR-5096 were issued for engineering evaluation. All twenty-two accumulators were inspected, cleaned, and desiccant removed as required.

The rust / scale formation discovered in the 6 accumulators discussed above has been removed by hydrolazing.

However, QC documentation remains unavailable.

To resolve this concern, the accumulators' as-built conditions will be verified and documented and the accumulators will be inspected to assure the installations are in accordance with applicable installation criteria.

This activity will be completed by August 1, 1988.
