SUBJECT: NRC INSPECTION REPORT 50-445/97-09 50-446/97-09 AND NOTICE OF
VIOLATION

Dear Mr. Terry:
An NRC followup inspection of steam ganerator program activities and material condition
was conducted March 17-21,1997, at your Comanche Peak Steam Electric Station,
Units 1 and 2, reactor facilities. The enclosed report presents the scope and results
of that inspection.
Overall, the results of the inspection indicate that you have addressed weaknesses
observed during the original steam generator inspection (NRC Inspection Report 50-445;
-446/94-01) by development of comprehensive program requirements and initiatives for
detection of degradation in steam generator tubing. We consider the use in the last
refueling outage of conservative examination expansion criteria and adoption of new eddy
current examination technology for examination of hot-leg side expansion transitions to be
both commendable and indicative of management support for steam generator tube
integrity init;atives. One violation was identified, however, pertaining to the failure to
require the use (during acquisition of plus point probe examination data) of probe and
extension cable types and lengths that were consistent with those specified in the
procedure that was qualified in acccrdance with Appendix H of the Electric Power
Research Institute, "PWR Steam Generator Examination Guidelines." This issue is of

concern since we believe that it is an indicator of an area where oversight of contracto;

performance may require further actions.

The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation is described in detail in the enclosed report. Please note that you
are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
