e HKM Associates License No. 25-17492-01 Notice of Violation Based on the results of the NRC inspection conducted on August 7, 1978, certain activities appear to be in noncompliance with NRC regulations '
and the conditions of your NRC License No. 25-17492-01 as indicated below:
- 1. Contrary to license condition 12, an individual employed by the licensee was purportedly using licensed material without having successfully completed the gauge manufacturer's training course or in the physical presence of an individual designated by the Radiation Safety Officer that had successfully completed the course.
This is an infraction.
- 2. Contrary to license condition 13, two sealed sources, Cs-137, 8 mci, Serial Number: CC1765 and AM-241: Be, 40 mci, Serial Number: CAA763, were not tested for leakage at intervals not to exceed six months during the period May 31, 1977 to May 9, 1978.

- 3. Contrary to license condition 17, the doors to the storage area, where the sources noted in item 2 above were stored, were not locked to keep people from freely entering as described in item 14 of license application.
This is an infraction.
- 4. Contrary to 10 CFR 20.203(e), the storage area noted in item 3 above was not posted with a sign bearing the radiation caution symbol and the words: CAUTION (or DANGER) RADI0 ACTIVE MATERIAL (S).
The inspector noted corrective action on this item during the inspection. ;
This is a deficiency.
- 5. Contrary to 10 CFR 30.51, records of receipt of the sealed sources noted in item 2 above, were not maintained on August 7, 1978.
