Dear Mr. Fredrickson:

In your phone call of March 9 you requested we identify all informtion in the incident report that we considered confidential. In order to avoid possible invasion of privacy and public embarassment to any individual, all names in the incident report should be deleted before a copy is placed in the public document room. Deletion may be achieved either by turking out the name or removing the entire document.

We believe that deletion should occur in all text, memos, letters, exposure rect.rds, conclusions, or disciplinary actions. The only exceptions are when a si9 nature appears on an official document tc signify completion, accuracy, or an r other certification. The two technical papers at the end of the incident report are previously published and this restriction does not apply.

Your attention it drawn to 10CFR9 paragraphs 4, 5, and 6; specifically, the requirements for maintaining personal privacy.
