Administrative Procedure (AP) 00410-C (Revision 2),

Computer Software Control, established controls for computer software important to safe and reliable plant operation.

Contrary to the

above, AP 00410-C did not establish controls over-supplied (sic) constants, which are distinct from the controlled 4

programmer-supplied constants.

As a

consenuence, on or about Dece*2er 10, 1987, the PROTEUS computer data bast 1 accessible constants was changed to include a non-conservative substitute value for blowdown flow.

In the event of a failure of a blowdown flow measuring channel, power indicated at computer point Ull18 would have been lower than the actual power level.

The unit is operated based upon the display value of Ulll8 in accordance with Standing Order 1-87-70.

U1118 is a'so used in Surveillance Procedure 14030-1, Power Range Calorime tr', c Channel Calibration.

This is a Severity Level IV violation (Supplement I)."

The violation occurred.

Reason for the violation:

The violation occurred due to a procedure inadequacy in that Procedure 00410-C, Rev.

2.. "Computer Sof tware Changes," did not contain provisions to control data base constants changed by plant personnel.

1) By the end of the NRC inspection, steps were taken to change the aefault value of blowdown flow from 90 gpm to 0 gpm prior to the restart of Vogtle Unit 1, and l2) To prevent unreviewed or unauthorized changes to the data base constants used in the calculation of U1118 (computer monitoring data point), the access level to these data base constants was changed to T

the highest level (Level 8) on December 18, 1987.

Since that time,

only the computer engineer has had access to the data base for constants affecting U1118.

Corrective steos which will be taken to avoid further violations:

Procedere 00410-C will be revised to include a review of "given data" for accutacy and to provide control of user-speci fi ed constants.
