Dear Mr. Kalman:

Per your request, this letter provides additional information regarding Administrative Procedure AP.28, "Post Trip Transient Investigation, Assessment, and Reporting." Administrative Procedure AP.28, adapted from INPO Good Practice OP-211, "Post-Trip Reviews," is consistent with industry practice. AP.28 provides event type definitions to assure the appropriate level of technical and management review prior to reactor restart following an event.

In the case of a Type I event, classified by consensus of all members of the Shift Management, the cause of the trip is known and no safety-related or other important equipment functioned in an abnormal or degraded manner. AP.28 provides detailed requirements for data collection, personnel statements, transient analysis and evaluations, and trip classification and startup approval. The Director, Operations and Maintenance, or his designee, is responsible for approving reactor startup following a Type I event.

In the case of a Type II event, the cause of the trip is unknown, or safety-related or other important equipment functioned in an abnormal or degraded manner. For Type II events, in addition to the extensive data collection and analysis performed for Type I events, AP.28 requires that a team or teams be appointed to perform the further investigation of the trip to determine necessary corrective action before restart. The team (s) will consist of adequate technical expertise which may include nuclear steam supply vendors, vendor engineers, onsite engineering staff, corporate engineering staff, and other experienced operations and maintenance personnel. The Director, Operations and Maintenance, or his designee, is responsible for approving reactor startup following a Type II event and for assuring adequate measures have been implemented to prevent repetitive challenges to safety systems during future power operations.

i AGH/NPP 88-192 George Kalman The Rancho Seco management is firmly committed to implementing the existing, approved Administrative Procedure AP.28. In recent months, the management team has clearly demonstrated that testing and progress toward restart will be delayed when broader implications of personnel errors and equipment malfunctions have not been adequately evaluated and absorbed. AP.28 requires the Independent Investigation / Reviews Group to perform a detailed evaluation following each plant event to examine lessons learned for improving plant safety and reliability. Rancho Seco management is committed to maintain a rigorous approach to nuclear safety, which includes thorough post-trip reviews, following plant restart from the present outage.

A copy of AP.28 is athched for your review and evaluation.
