NRC Bulletin Item 1:
Describe a) the characteristics currently examined during receipt inspection of fasteners (i.e., head markings for grade and manufacturer symbols, review of certified material test report or certificate of conformance), and b) internal controls utilized during storage and issuance from stock to assure the appropriate use of fasteners.
Response
la) All receipt inspections of bolts are currently performed under the control of a Quality Assurance receipt inspection plan which enhanced the standard receipt inspection procedure. This plan assures that the following characteristics are inspected a) Manufacturer's symbol b) Gre e designation marking c) Packaging / storage of fasteners d) Segregation of lots / heats e) Fastener identification f) General physical condition Documentation required by the purchase order (Certified Material Test Reports, Certificate of Conformance, etc.) is revieved for acceptability in accordance with the appropriate industry code or standard. Any bolts which fail to meet any of the above characteristics are placed in a OA i
hold area pending resolution.
All nuts are inspected for the standard receipt inspection attributes of identification, marking, shipping damage, packaging and shipping requirements and physical condition (physical damage, cleanliness, size and configuration and coatings and preservatives). When required to support commercial grade dedication, special receipt inspection plans are generated for establishing acceptance criteria.
Documentation required by purchase orders for nuts is reviewed in the same fashion as for bolts.
Ib) During the purchasing process a computerized stock code is established to identify the item being purchased. This stock code is used throughout the i
procurement, receipt, storage, issuance, and installation of the purchased fastener.
After a satisfactory receipt inspection, the fasteners are released to the parts varehouse for storage. All safety related and augmented safety related fasteners are tagged by Quality Assurance with a Material Conformance Tag containing at least the material receipt number, and a

stock code number.
Fasteners are typically stored as received from the vendor (boxed or in bags and with thread protectors, if supplied) and in accordance with ANSI N45.2.2 guidelines. Varehouse identification on the storage container includes the stock code number, the material receipt

number, the item description and the purchase order number.
Upon receiving requisitions for material, Material Services Section personnel verify the material availability in the varehouse.
Safety-Related, Augmented Quality and ASME requests require evaluation prior to issue. The material requester checks the Engineering Evaluation Data Base to determine if the stock code has already been evaluated and approved for the requested application.
If it has been evaluated the item is acceptable for issue.
If the stock code has not been pre-evaluated, an evaluation which includes a review of design configuration documents (including Equipment Qualification reports, as applicable) is performed to determine that the proper part was requested for issuance.
Internal controls of storage and issuance also include routine Quality Assurance audits and surveillance of the complete varehouse storage, handling and issuance program.

Enclosuro PY-CEI/0IE-0299 L Page 3 of 8 NRC Items 2 & 3 Select a minimum sample of ten (10) non-safety related fasteners (studs, bolts, and/or cap screvs), and ten (10) safety-related fasteners (studs, bolts, and/or cap screvs) from current, in use, stock. The sample is to be obtained by the licensee with the participation of an NRC inspector.
Fasteners procured to meet the following chemical and mechanical properties are of interest:
A-193 grades B7, B8, and B16; SAE J429 grades 5 and 8 A-449; A-325 Types 1,2 or 3 A-354 grades BB, BC, BD; A-490 A-320 LTH; A-307; A-563; or equivalent.
For the selected sample of fasteners in item 2, include a sample of typical nuts that vould be used with each fastener (one-for-one).
In particular, nuts purchased to the chemical and mechanical specifications of A-194 are of interest.
Response
The sample selection for PNPP is listed below and shovn in Attachments 1 and 2.
The NRC Senior Resident Inspector selected the items to be used in the testing program. A minimum of three samples of each item was tested.

f NRC Item 4 Chemical testing shall be performed on all samples. Mechanical testing shall be performed on each safety-related fastener. Hardness testing shall be performed on each nut and non-safety-related fastener. All testing shall be performed by a laboratory which the licensee has qualified for this type of testing and appears on the licensee's approved vendor list. Testing performed shall be done in accordance with the requirements of the fastener's specification, grade, and class, and the test shall evaluate the ultimate tensile strength, hardness and chemical properties as required by the fastener's specification, grade, and class.
Each sample shall be tagged with the sample's ID number.
l Responset Chemical testing, mechanical testing, and hardness testing have beer, performed on all of the samples. All testing was performed by an approved laboratory quaTIlied for this type of testing. Testing was done in accordance with the requirements of the fasteners specification, grade, and class, and the tests have been evaluated for the ultimate tensile strength, hardness, and chemical properties as required by the fasteners specification, graue and class. j fasteners purchased to ASME Section III requirements, the additional tests required by Section III (impact tests) vere also performed. The results of 1
this testing are tabulated in Attachment 2.

6 The results of all tests, together with supporting information, are to be reported to the NRC utilizing the format shown in Attachments 1 and 2 of this bulletin. Include the names and addresses of suppliers and manufacturers of j
safety-related fasteners and, to the extent possible, of non-safety-related fasteners.
For any fastener found out of specification, provide an evaluation of the safety significance including concideration of the most limiting application.
Response
The results of all of the tests, together with the supporting information requested is contained in Attachment 1 and 2 of this letter, using the format depicted in the bulletin. One fastener and one nut did not ineet the testing specifications. A brief description of each and an evaluation of the safety significance is provided below.
a)
Sample Number 11 consisted of 1/2-13 X 2" Hex Headed Cap Screws made of ASTM A307 Grade A material. The bolts vere procured non-safety, ano it has been verified that none of these bolts were used in any safety applications. The test results indicate that the Hardness (22 HRC) is t
very slightly sbove specification requirements. Other properties me* or exceeded the specification requirements (101 HRB actual versus 100 HRB
' required by specification). The review of issuances indicate 100 of these fasteners were issued.
Since these bolts have not been used in any safety application and the results vere acceptable for the critical parameters including the tensile and yield stresses, there is no safety significance i
to the noted discrepancy.
1 b)
Sample Number 12 vere 1/2".13 Heavy Square Nuts made of ASME SA 194 Grade i
2H material. Although these nuts vere purchased as safety related, they
[
vere included in the bulletin testing program as a non-safety related nut because of insufficient numbers of non-safety related nuts in stock. The nut was tested for all ASME specification requirements. Two of the three i
nuts tested shoved a slightly lover (20 HRC and 23 HRC) Hardness value than the specification minimum (24 HRC). All other test results including F
the proof load met the specification requirements. Since the results vere l
acceptable for the critical parameter (proof load) and a teview of the purchase order and it,suance docuunts revealed that none of those nuts a
have been used, there is no safety significance.
During the conduct of the testing program, three discrepancies vere identified in the elongation results. ASTM A307 Samples number 11 and 25 vere tested i
{
using both machined specimens and full size bolt samples. In accordance with ASTM A307, when bolts are tested by both full size and by machine test specimen methods, the full-size test shall govern if a controversy between the two t
methods exists. The machined specimen tests for these tvo samples resulted in i
elongation values less than the specification minimum. However, the full size bolt testing for both samples resulted in acceptable ultimate tensile i
strengths. Therefore, per the ASTM A307 requirements, the bolts are acceptable except for the slight hardness discrepancy of Sample 11 described in item "a",
above. The results of this testing is provided in Attachment 2.
- Further,

Enclosure PY-CEI/0IE-0299 L Page 7 of 8 initial tests of Sample 39 indicated that elengation was slightly less than the ASME SA 193-B7 minimum.
Subsequently, Samr!.e 39 was retested with twice the number of samples, as permitted by tae standard. These retest results (see ) were fully acceptable. Even though the bolts in these three samples met all Standard requirements, all field applications vere reviewed.
An evaluation of the applications shoved that the bolts were not used in any fatigue-type loading situations.
Based on all the above, there is no safety significance to the minor elongation discrepancies.
Also during the conduct of the testing program, two discrepancies with stock coding were identified.
Sample Number 5 bolts were originally identified as ASTM A193 Grade B7. However, the markings and test results both indicate the fasteners were, in fact, SAE J429 Grade 5.
These fasteners were incorrectly categorized upon field return to the varelo se.
Seven of these miscategorized bolts were subsequently issued to the field for a SAE J429 Grade 5 bolt or better request. Although the bolts vere miscategorized, the material issued was acceptable for that application. The other stocking code problem dealt with sample Number 36. The stock cede description indicated that the non-safety related bolts were SAE J429 Grade 5.
Hrvever the bolts were marked and shown by testing to be ASTM 193 Grade B7.
Properties of A 193 Grade B7 exceed that of SAE J429 Crade 5.
The A193 Grade B7 in stock is an acceptable replacement for any SAE J429 Grade 5 application.
All the fasteners / nuts in the varehouse with the ab,ve described discrepancies have been placed under the norconformance control rrogram and have been physically segregated in the QA hold area.

Based on the result of the testing and review of current procedures, describe any further actions being taken to assure that fasteners used in the plant meet the requisite specifications and requirements and that the operability of safety-related plant components is not affected.
Response
The Cleveland Electric Illuminating Company has been engaged in a review of the fastener identification and testing program since September 1987. A comprehensive inspection and testing program was developed to perform enhanced receipt inspections and complete mechanical and chemical property testing of selected fasteners in the spare parts inventory. This continuing program is broader than the required testing performed in response to the NRC Compliance Bulletin 87-02.
This enhanced receipt inspection plan and testing program are part of further actions being taken to assure fasteners used at Perry meet their requisite specifications requirements. The enhanced receipt inspection plan has been utilized to reinspect all safety and non-safety bolts in the spare parts inventory to verify the characteristics identified in our response to Bulletin Item la.
The reinspection, testing and evaluation of results to date are comparable with those provided herein.
The conclusions to date indicate no safety significance due to any bolt discrepancies.
In addition to the technical evaluation of any individual testing discrepancies, further evaluation of the application and documentation associated with each is being performed in accordance with the Quality Assurance. Program to determine final disposition.
As a result of non-safety related stock coding discrepancies such as those identified in Samples 5 and 36, ve have reviewed all non-safety related fastener data base descriptions against the procurement documents, and performed a physical inspection to verify that the fasteners in inventory agree with the stock code data base. All noted discrepancies are being evaluated and necessary corrective actions are being initiated. No concerns with the safety related stock coding program have been identified. This is due to the safety related fastener receipt inspection by QA vhich verifies correct stock code descriptions, and tagging of acceptable fasteners by QA prior to placing the fasteners into inventory.
Additionally, ve are in the process of defining a limited number of bolt suppliers based on the results of the above described evaluations.
Distributors found supplying fasteners with suspect markings vill be ruvieved for elimination as a fastener supplier.
Also an interim engineering review has been added to all nonsafety related bolt purchase requisitions, to assure that the proper fastener material is being purchased. This vill continue until the total program enluation is complete.
Based on our inspections and reviews performed to date, we believe our current programs assure the fasteners CEI has procured for the Perry Nuclear Power Plant meet the necessary requirements to assure that operability of safety related components is not affected.
