Thank you for your letter dated September 13, 1978, in which you reported an incident involving lost sources of Technetium-99m, in the form of three diagnostic doses totaling 25 mci in a shielded container, as required by paragraph 20.402(b), Title 10 of the Code of Federal Regulations, Part 20.
The circumstances surrounding the incident of the lost Technetium-99m and your corrective actions have been reviewed by the office.
The position of this of fice is that, in addition to the actions described in your letter "l
dated September 13, 1978, your Management Controls of this licensed progrr, should be improved so that the RPO, his designee, or other management repre-sentative will be immediately notified of any incident involving licensed material as specified in paragraphs 20.402 and 20.403, Title 10 of the Code of Federal Regulations, Part 20. The occurrence of any such event in the future should be reported to the NRC in accor dance with these paragraphs.
If further information is required, we will contact you. We will examine j
your corrective action during a future inspection.
