50-328 SEQUOYAH NUCLEAR PLANT (SQN) - REVISION TO RESPONSE TO NRC INSPECTION REPORT NOS. 50-327/87-50 AND 50-328/87-50 In response to a verbal request from the SQN resident inspector, I am submitting this revision to my November 4, 1987 response to NRC's concerns relative to violations 50-327/84-24-01 and -02.
This submittal addresses only example e of violation 50-327/84-24-01 and reflects current procedural requirements regarding hold orders and caution orders.

Administrative Instruction (AI) 3, revision 40, dces not allow the use of caution orders for personnel safety.
Section 3.1.5.3 states that, "A caution order will not be used as a clearance boundary or in-plant for protection and I
safety of personnel." Under plant emergency conditions, the shift supervisor may authorize the removal of hold notices and operate equipment under his control to place the plant in a safe condition.
The shift supervisor returning equipment to service under these conditions is responsible for the safety of plant personnel.
AI-3, section 5.3.1.2, requires that, under normal conditions, no work begin on the subject equipment until the clearance has been issued to the person responsible for the work. Additionally, clearances may only be issued to properly authorized employees whose names appear on the official clearance i
list.
The shift supervisor may hold a clearance for emergency maintenance purposes or for manufacturers' representatives or TVA personnel on special assignment.
