NOTICE OF VIOLATION (NRC INVESTIGATION REPORT 4-96-004)
Dear Mr. Gracin:
The enclosed Notice of Violation was sent to you by certified mail on December 19,1996, and was returned unclaimed to the Nuclear Regulatory Commission in January 1997. The enclosed was sent to the same mailing address used for previous correspondence, i.e., a September 11,1996, letter to you, which was signed for as received at the above address.
The purpose of this letter is to inform you that this is your last opportunity to respond to the enclosed Notice of Violation. The 30-day period in which to respond will begin from the date of this letter. If you do not respond, the NRC will consider this matter closed and will consider its action final.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, with your home address removed, its enclosure and any response you choose to submit will be placed in the NRC Public Document Room.
