l In accordance with your application of January 30,1996, the U.S. Nuclear Regulatory Commission issued Quality Assurance Program Approval No. 0846 (QA-0846) for use of Transportation Packaging Model DOT 6M. The QA-0846 had an expiration date of j
February 28,1997. NRC licensees, including QA holders, are subject to annual fees under 10 CFR Part 171 of the Commission's regulations. However, the licensees participating in the DOE /LANL Plutonium Beryllium Neutron Source Recovery Program have been exempted from annual fees. Therefore, Annual Fee invoice No. AM2098-97 (copy enclosed) is hereby cancelled. We received your payment of $950.00, which will be refunded. You should be receiving the refund in approximately 30 days from the date of this letter. Thank you for your cooperation in this matter.

Notice is hereby provided that your Quality Assurance (0A) Program Approval for Radioactive Material Packages. Approval No. 0846, expired on February 28, 1997.
If you are an NRC licensee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71, or an Agreement State Licensee subject to the requirements of 10 CFR Part 71 as required by 10 CFR S 150.20 a OA Program approved by the Commission as satisfying the provisions of Subpart H of 10 CFR Part 71 is required.
Since you no longer have a valid 0A Program Approval.
the General Licenses of Subpart C of 10 CFR Part 71 and registration under the provisions of 10 CFR S 71.12. to transport or deliver licensed material to a carrier for transport are terminated.
Please note that records required by 10 CFR SS 71.91 and 71.135 are to be retained for a period of three years.
