Wadley, Vice President ' Nuclear Generation Northern States Power Company 414 Nicollet Mall Minneapolis, MN 55401 SUBJECT: NRC INSPECTION REPORT NOS. 50-282/97005, 50-306/97005, 72-10/97005, AND NOTICE OF VIOLATION Dear Mr. Wadley: On April 4,1997, the NRC completed an inspection at your Prairie Island Nuclear Generating Plant. The enclosed report presents the results of that inspection. Command and control of plant evolutions were generally acceptable during most of this inspection period. However, we are concerned about an inadvertent reactivity addition which occurred on Unit 2 during the beginning of the next inspection period. This represents the second occurrence of unintentional core reactivity addition within four months time. Also during this period, excessive inventory was inadvertently drained from the reactor coolant system. A management meeting has been scheduled with you in the Region lil office on May 20,1997, at 1:00 p.m. to discuss these and other conduct of operations issues. Based on the results of this inspection, the NRC has determined that two violations of NRC requirements occurred. The violations are cited in the enclosed Notice of Violation (Notice) and the cucumstances surrouncing them are described in detail in the subject inspection report. The first violation is for a failure to follow procedures, with two examples. Control room operatorc deliberately placed Unit 2 in Technical Specification 3.0.C to provide a 1 hour allowed outage time to isolate the safety injection system from the reactor coolant , system and fill the accumulators. The operators did not understand that this was a condition prohibited by tha Technical Specifications. The second example pertained to valve manipulation beyond the scope of a surveillance procedure. For the other violation, a Prairie Island security contractor supervisor deliberately falsified a record required by NRC regulations to cover up an error he made. A Pre-decisional Enforcement Conference was conducted on March 18,1997, to discuss the apparent violation identified in Inspection Report 282(306)/97002 involving control of heavy loads (EA 97-073). You will be informed of our decision regarding enforcement k action via separate correspondence. $ You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response.
