Dear Mr. Mcdonald:
In accordance with my conversation with your office I am hereby requesting that we be registered as a user for transport of certificate of compliance 4888 and certifica,te of compliance 9030.
We have acquired title to the following items named in the certificates and wish to transport them to our facility and ultimately to another licensee.
The units involved are:
1-Sentinel LCG25BRTG-3-Sentinel 3 (MW 3000) RTG (S/N 2,4,5)
We are presently in possession of the following:
1.
Certificate of compliance 4888 2.
Certificate of compliance 9030 3.
Safety analysis used for obtaining the certificates.
4.
Safety evaluation. covering the use of the RTG units in undersea operations.
5.
Valid State of Texas license # 8-1811 authorizing possession of the RTG units.
As of June 1,1977 authorized users of permit 4888 were:
