NRC Inspection No. 78-15 Shoreham Nuclear Power Station - Unit No. 1 Docket No. 50-322

Dear Mr. Carlson:

This letter responds to your letter of October 17, 1978, which forwarded the report of the inspection of activities author-ized by NRC License No. CPPR-95, conducted by Mr. Toth of your office on September 26-29, 1978.

The letter stated that it appeared that one of our activities was not conducted in full compliance with NRC requirements.

The apparent noncompliance and our response follow:

Apparent Noncompliance with 10CFR50, Appendix B Criterion V, and FSAR Section 17.1.5A Contrary to the above, on September 29, 1978, S&W field forces performed kelding on a Skewed Tee Joint of angle 54 degrees, on the rectangular tubular pipe support number lEll-PSA-072 of the residual heat removal system, using procedure W70G which was pre-qualified only for weld joint configurations having angles greater than 60 degrees.

Corrective Action and Results All previously accepted tubular pipe supports having

Skewed Tee Joints were reviewed, and those having an angle less than 60 degrees have been identified.

A welding procedure incor-porating the essential variables of Technique W70G, but with ex-panded angular 'colerances, is being qualified.

Successful qualification of this procedure will provide justification for accepting those welds with angles less than 60 degrees identified in the above review.

I Steps Taken to Prevent Recurrence Field Quality Control hanger inspection personnel have been reinstructed to verify the use of properly qualified welding techniques during their in-process inspections, and Construction personnel have also been advised of this problem to insure that correct weld procedures are used on future hanger installations.

Further, craft and suoervisory contractor personnel are receiving additional training t-insure that all personnel involved in hanger welding understand the importance of utilizing the corrret qualified welding techniques.

Inspection 50-322/78-15' This refers to the inspection conducted by Mr. A. Toth of this office on September 26-29, 1978 at the Shoreham Nuclear Power Station at Shoreham, New York of activities authorized by NRC License No. CPPR-95 and to the discussions of our findings held by Mr. Toth with you at the conclusion of the inspection, and to a subsequent telephone discussion between Mr.

Toth and Mr. J. Kelly on October 3,1978.

Areas examined during this inspection are described in the Office of Inspection and Enforcement Inspection Report which is enclosed with this letter. Within these areas, the inspection consisted of selective examinations of procedures and representative records, interviews with personnel, measurements made by the inspector, and observations by the inspector.

Based on the results of this inspection, it appears that one of your activities was not conducted in full compliance with NRC requirements, as set forth in the Notice of Violation, enclosed herewith as Appendix A.

This item of noncompliance has been categorized into the levels as described in our correspondence to you dated December 31, 1974.

This notice is sent to you pursuant to the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2 Title 10, Code of Federal Regulations.

Section 2.201 requires you to submit to this office, within thirty (30) days of your receipt of this notice, a written statement or explanation in reply including:

(1) corrective steps which have been taken by you and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance I

will be achieved.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter and the enclosures will be placed in the NRC's Public Document Room.

If this report contains any infonnation that you (or your contractor) believe to be proprietary, it is necessary that you make a written application within 20 days to this office to withhold such information from public disclosure. Any such application must be accompanied by an affidavit executed by the owner of the information, which identifies the document or part sought to be withheld, and which contains a statement of reasons which adoresses with specificity the items which will be considered by the Commission as listed in subparagraph (b) (4) of Section 2.790.

The information sought to be withheld shall be incorporated as far as possible into a separate part of the affidavit.

If we do not hear from you in this regard within the specified period, the report will be placed in the Public Document Room.

APPENDIX A NOTICE OF VIOLATION Long Island Lighting Company Docket.No. 50-322 Tnis refers to the inspection conducted by representatives of the Region I (Philadelphia) office at the Shoreham Nuclear Power Station, Shoreham, New York, of activities authorized by NRC Construction Permit CPPR-95.

During this inspection conducted on September 26-29, 1978, the following apparent item of noncompliance was identified.

10 CFR 50, Appendix B, Criterion V requires, in part, that "Activi-~

ties affecting quality shall be prescribed by documented instruc-tions, procedures,...and shall be accomplished in accordance with these instructions, procedurcs...."

The Shoreham Nuclear Power Station TSAR Section 17.1.5A requires that suppliers of safety releted materials and services are responsible for imposing the above requirements on their internal operations.

The implementing S&W Quality Assurance Program Manual for Shoreham, Section 9, describes that the welding procedure specifications

"...have been qualified in accordance with applicable codes and standards and are the written procedures followed for erection work performed by S&W field forces."

S&W Welding Procedure Technique Sheet W70G Revision 0 is prequali-fied in accordance with applicable code AWS Dl.1-72 for fillet welds in Skewed Tee Joints of angles greater than 60 degrees.

Contrary to the above, on September 29, 1978, S&W field forces performed welding on a Skewed Tee Joint of angle 54 degrees, on the rectangular tubular pipe support number 1 Ell-PSA-072 of the residual heat removal system, using non-applicable procedure W70G which was not prequalified for the weld joint configuration.

This item is an infraction.

Inspection on September 26-29, 1978 (Report No. 50-322/78-15)

Areas Inspected: Routine, unannounced inspection by two regional based inspectors of Work Activities for reactor vessel feed water nozzle rework, ano fuel' pool weld inspection / testing.

The inspectors also performed plant tour-inspections, reviewed licensee action on previous inspection findings, and performed follow-up action as necessary to resolve questions which arose during the course of inspection of the above specific areas.. The inspection involved 52 inspector-hours onsite by two NRC regional office based inspectors.

Results: Of the two specific areas inspected, no items of noncompliance were identified; one apparent item of noncompliance was identified during the plant tour-inspection (Infraction-failure to use qualified procedure for welding pipe support-Paragraph 5.)
