Dear Mr. Davis,

l Re: Your letter of November 3,1978 Indiana University item of non-compliance License SNM - 654 Dr. Koch, Chairman of the Radiation Safety Committee, asked me to respond to your letter since he will be gone for the next two weeks.

The response is as follows:

1.

At the time of the inspection, the plutonium-heryllium source had been removed from the storage location in the modern physics laboratory by a person not approved by the Radiation Safety Committee.

Corrective action involved immediately returning the source to the locked storage area and posting a note on the source shield that it was not to be removed or used by unapproved per-This was donc, at the time of the inspection.

The corrective action taken at the time was for the purpose of avoiding l

a future non-compliance.

In addition, a copy of this letter is being forwarded to faculty and selected staff in the Physics Department.

Full compliance was achieved at the time of the inspection.
