REQUEST FOR ADDITIONAL INFORMATION CONCERNING A MATERIAL LICENSE APPLICATION (
REFERENCE:
CONTROL NUMBER 251613)
This refers to your application dated May 29, 1987, for a license to possess and use portable gauges containing licensed material. Additional information is needed in support of your application.
Enclosed for your guidance is a copy of our licensing guide for portable gauges, Task FC 407-4. We have referenced pertinent items in this guide which you should review and provide information.
Please provide the following information:
1.
Training:
Please refer to Item 7 in the guide and provide information describing the training your designateu Radiation Safety Officer has received.
As a minimum he should have completed the device manufacturer's training program.
2.
Training:
Please refer to Item 8 in the guide and provide a commitment that each employee will complete the device manufacturer's training program before they are permitted to use the device.
3.
Facilities and Equipment:
Please refer to Item 9 in the guide and provide a sketch of the storage facility and how it is secured to prevent access by unauthorized persons.
4.
Personnel Monitoring: Refer to Item 10.1 and provide the name of the supplier of your personnel dosimeters, the type of dosimeter (film badge or TLD) and exchange frequency.
5.
Waste Management: Please refer to Item 11 in the guide and state how you will dispose of depleted sources.
If, for example, you will return the sources or gauges to the supplier.
Our review of your application will continue upon receipt of the above information.
