HIGHLIGHTS OF WEEKLY INFORW. TION REPORT WEEK ENDING JANUARY 15, 1988

. Indian Point 2 Indian Point 2 is in the process of starting up after the recent refueling outage. During the heat up of the plant and before the primary system reached 350'F, it was discovered that steam generator #23 had boiled dry. The level in the steam generator began decreasing on January 1,1988, at 11:30 p.m. and had boiled dry by 8:00 a.m. on January 3,1988 as determined by a review of the logs. It was January 4 before the plant realized that the steam generator was

' dry and another day before the steam generator was refilled. It appears that the operating staff was continuously aware of the decreasing water level. The NRC has an Augmented Inspection Team, including the Project Manager, arriving at the site on Janucry 8.

Following the Stean Generator Dry Out Event and as documented in a Confirmatory Action Letter issued on January 7,1988, Consolidated Edison agreed not to take the plant critical until authorized by the Regional Administrator. The Augmented Inspection Team (AIT) dispatched to Indian Point 2 on January 7 to investigate the event held an exit meeting on January 12. Based upon the results of the inspection, the licensee was inforced .iat several action items must be completed

, prior to the Regional Administrator authorizing restart. First, Consolidated Edison must submit a letter to the NRC discussing short-and long-tem corrective actions. Second, the utility must meet with the NRC staff to discuss the action plan. The utility must demonstrate the rationale used in determining its corrective actions. It must demonstrate the effectiveness cf short-tem actions and justify why long-term actions should not be completed prior to restart.

NRC/ DOE Physical Security Comparability - Night Firing Qualifications NMSS staff has confimed that as of January 10, 1988, all licensees have completed night firing qualifications for their security guards. License conditions to implement the three near tem NRC/ DOE comparability measures

, (stationing armed guards at MAA portt.ls, night firing qualifications for guards, and conducting 100% search of personnel and packages entering the protected area) were issued on August 6, 1987, to the four Category I fuel facilitylicensees(NFS,B&W,UNC,andGA). The stationing of amed guards at MAA portals was previously fully implemented at all four facilities on September 9, 1987. The implementation of the 100% search requirement for all personnel and packages is to be accomplished on a phased implementation schedule (25% search by November 9, 1987, 50% seaN5 by March 9, 1988 and 100%

search by August 9, 1988). GA and NFS have alreedv implemented the 100%

search while B&W and UNC are on schedule for the phased implementation.

Secretarial Job Fair The Office of Personnel sponsored a secretariel/ clerical recruitment Job Fair on Saturday, January 16. Approximately 50 applicants were interviewed by the six-member special OP recruitment team, supplemented by a representative of HMSS. The overall quality of the applicants was high, and the majority of those who successfully passed the typing test will be referred for possible selection threughout the agency. This was the most successful Job Fair the agency has sponscred in some tine; reasons for the high attendance appeared to be the attraction of the White Flint location and the relatively high job security of Federal employment for secretarial and clerical personnel. OP will attend another professionally-sponsored clerical job fair on February 3 at the Sheraton Potomac.

the site on January 8. 1 PILGRIM - Restart Hearings by Senator Kennedy On Jariery 7,1988, Serator Kennedy, as Chairman of the Senata Labor and Human i Resources Comittee, held hearings regarding the restart of the Pilgrin Nuclear Powt:r Stat'on. These hearings were held in the Plymouth High School i before a crowd of about 500 individuals.

Senator Kennedy received testinony from the following panels and individuals:

- Leaders of local citirrrs groups I

- Massachusetts and local elected officials

- Massachusetts Lt. Governor and Attorney General

- Connonwealth Emergency Planning and Public Health Officiels

- NRC and FEMA The major issues of the hearing concerned energency preparedness in the Pilgrin area, public health issues in the Pilgrin area, the adequacy of the Mark I Containmort at Pilgrim and desires to hold formal adjudicatory hearings. i Senator Kennedy indicated that followup questions may be directed to the NRC.

Shnreham On January 7, 1988 the ASLB (OL-6) issued the following rulings related to LILCO's request for authorization to operate Shorehan at 25% of full power:

(1) LILC0 is entitled to proceed with its request under 10 CFR 50.57(c)

LILCO is entitled to apply the without first seeking an exenp(tion.to provisions in 10 CFR 50.47(c) assurance" recuirements of 50.5/ 1)(a)(satisfy the "reasonable 3)asrequiredunde (2) The intervenors are entitled to contest LILCO's request as provided for under 50.57(c).

(3) The Staff is directed to proceed with a review of LILCO's request.

The Staff's Safety Evaluation Report should be made available to give the intervenors a further opportunity to state their contentions with basis and specificity.

(4) In light of the above rulings, the parties are directed to recommend 1 on the appointment of a separate Board or other special assistants as l provided for under 10 CFR 2.722. I Beaver Valley 1 The unit has been shut down for refueling since mid-December. On performance  !

of examination of radiographic examination on the feedwater lines, licensee  ;

personnel discovered indications of cracks on the nozzle-to-fitting welds of 1 all three lines. These cracks, ranging frcm 17/8" to 81" around the l circurrference of these lines, are located at the counterbore areas. The j affected portions of the lines have been cut out and specircens of material '

hr.ve been sent to contractors to determine the nature and causes of these cracks. The licensee plans to replace all three affected elbows.

The elbows were last replaced in 1979, when cracks were discovered as a result of ections requested by IE Bulletin 79-13 (PPA B-43. TAC No.11766). NRR per;onnel (M. Hum and W. Hazelton) are closely following tMs event.

Mr. k' F. Conway. Mr. Conway was formerly President and Chief Executive Offi-cer of Vermont Nuclear Power Corporation ar.d recently held the position of i Group Vice President and Acting Division Director at the Institute of Nuclear j Plant Operations (INPO).

In the near future, FP&L will provide us the details of these and othcr organi-zational changes affecting both corporate and the Turkey Point site.

JANUARY 15, 1988 ENCLOSURE B

Crystal River Unit 3 Crystal River Unit 3 achieved criticality on January 8,1988 af ter a scheduled ,

refueling outage which began on September 19, 1987. Mcde I was reached on i January 10, and the plant is currently et 457 power in its power ascension '

program.

During the restart, three EFIC actuations of emergency feedwater occurred l caused by problems in the main feed system. No significant over coolirg j resulted from these actuations, but during one event the steam-driven EFW pump tripped on overspeed due to inadequate opening of the wamup valve.

Perry On Septenber 18, 1987, the Cleveland Electric Illuminating Company, et. al.,

licensees for the Perry Nuclear Power Plant, Unit No.1, requested a license amendment to remove the MSIV leakage control system from the Technical Specifications and to modify the allowable MSIV leakage fron 25 (SCFH) Standard Cubic Feet per Hour per line to 100 SCFH through any combination of lines.

Their technical basis for this request was a plant specific analysis of NUREG-1169 "An Evaluation of Boiling Water Reactor Main Steam Isolation Yalve Leakage and the Effectiveness of Leakage Treatment Methods" for the Perry Nuclear Power Plant, Unit 1. l l

This amendment reouest was noticed in the Federal Register on December 11, 1987 with prior opportunity for hearing without making a significant hazards determination in accordance with recent direction from NRR management. On January 8, 1988, Susan L. Hiatt, a representative for the Ohio Citizens for Responsible Energy (OCRE) filed before the Atomic Safety and Licensing Board, a Petition for Leave to Intervene and Request for Hearing with respect to the amendment. i Following the Steam Generator Dry Out Event and as documented in a Confimatory  !

License conditions to implement the three near term NRC/ DOE comparability measures (stationing armed guards at MAA portals, night firing qualifications for guards, and conducting 100% search of personnel and packages entering the protected area) were issued on August 6,1987, to the four Category I fuel facility licensees (NFS, B&W, UNC, and GA). The stationing of armed guards at MAA portals was previously fully implemented at all four facilities on September 9, 1987. The implementation of the 100% search requirement for all personnel and packages is to be accomplished on a phased implementation schedule (25% search by November 9, 1987, 50% search by March 9,1988 and 100%

searchbyAugust9,1988). GA and NFS have already implemented the 100%

search while B&W and UNC are on schedule for the phased implementation.

_TRANSRAM Workshops The Department of Transportation (DOT) sponsored a series of regional workshopsontheTransportationofRadioactiveMaterials(TRANSRAM)lastyear, and has proposed a second series of the workshops for this year. The workshops are held by the Department's Research and Special Programs Administration, with participation from NRC, the Department of Energy and the Federal Emergency Management Agency. Invitees include state and Indian tribal '

representatives with day-to-day regulatory and compliance responsibilities for radioactive material shipments. Basically, the workshops are intended to facilitate information exchanges and dialogue among state. Indian and federal agencies. The first workshop this year is scheduled for January 18-19 in Tampa, Florida. In response to a request from DOT, SGTB staff plan to participate.

INDUSTRIAL AND HEDICAL NUCLEAR SAFETY Dry Spent Fuel Storage A letter from Virginia Electric and Power Company (VEPCO), dated December 31, 1987, has stated its intent to request an amendment to ite Part 72 license to authorize storage in a different dry storage cask design at its Surry site.

Dry storage is presently conducted using the GNSI CASTOR V cask with a capacity of ?1 assemblies. The amendment, to be submitted in May 1988, is in JANUARY 15, 1988 ENCLOSURE C I , _ _ _ _ _ _ _ - - _

2 conjunction with the expected submittal in March 1988 of a topical report by GNSI for its nodular cast iron CASTOR X/28-33 cask design. The CASTOR X is designed to store 10-year-decayed spent fuel and has 2 fuel baskets. One basket design allows a cask capacity of 28 PWR assemblies. The second basket ,

design takes allowance for burnup credit in its criticality design and allows I a cask capacity of 33 PWR assemblies. VEPCO's action is part of a joint Department of Energy / Virginia Power / Electric Power Research Institute Dry Storage Cask Demonstration Program. j AlChemIE NRC staff met with DOE staff and its contractor, Martin-Marietta, on January 6, 1988, to discuss nonproliferation aspects of the AlChemIE proposal to operate two facilities for the enrichment of stable isotopes in Oak Ridge and Oliver Springs, Tennessee. NRC and 00E staff met with AlChemIE on January 7, 1988, I to discuss the same topic. Further discussions will be necessary to develop appropriate performance objectives. DOE also is considering this topic in connection with its contractual regulations with AlchemIE.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending January 15, 1988 REGULATORY DEVELOPMENT A-44, Station Blackout We have been working with the ED0 staff to ccmplete revisions to the A-44 resolution package. Concentrated cooperative effort provided by individuals from NRR, AEOD, OGC and EDO has resulted in revised wording to satisfy ED0 staff. We will provide comparative text to all offices for their review and approval this week, and if acceptable, we will provide the revised package to EDO 01/22/88.

Im) roved Risk Based Performance Indicator, and Proposed Rulemaking to Change LER Rule An important step in NRC's plan for perfomance indicators (SECY-86-317) is the development of an improved, risk based indicator of the unavailability of important safety system.

In December 1987, based on research results by BNL, DRPS recommended to AEOD a method for this improved indicator for unavailability of important safety systens. This improved indicator is: The total hours per year that any of six selected safety systems are unavailable while the reactor is critical.

This improved indicator for safety-system unavailability is directly related to safety. Six systems selected for monitoring represent about 90% of the sensitivity of core-melt frequency to outage of a safety-system train.

Also, trends in this unavailability indicator reflect the effectiveness of a plant's maintenance program in preventing the avail-ability of important safety systems from degrading. (Future work will provide other indicators of maintenance and better coverage of balance-of-plant.) A RIL is in t

preparation.

The data needed to implement this indicator are simple; e.g.: (1) The total number of hours that each train in the six selected systems is taken out of service,and(2)numberoffailuresofeachtrain. Ma g utilities infonnally track this type of information in order to assure meeting Technical-Specification limiting conditions for operation.

In order to obtain this insut data, an August ,18 nemorandum from AE0D to RES requested RES support to obtain EDO approval to initiate rulemaking. The RES response has been in concurrence since September. This memorandum has now been revised to more clearly state our reasons for recommending rulemaking. We expect AEOD Director concurrence this week, and will request

NRR Director concurrence next week.

D. Mackenzie, NMSS (301)427-4052 PREVENT CORE DAMAGE 2D/30 2D/3D is a trilateral (Japan, Gennany, and U.S.) experimental program. The NRC provides computer codes and two-phase flow instruments through INEL and ORNL in exchange for test data. The German Upper Plenum Test Facility (UPTF) is a full scale facility containing 193 fuel bundle upper end boxes and upper plenum internals but lacking a core heater. This program provides large scale test data on LOCA refill and reflood phenomena. These data serve as the best data source of revising 10 CFR 50 Appendix X rules.

Moreover, these data have been used to resolve the upper plenum injection (UPI)licersingissues. The NRC licensing staff have used the data to assess the emergency core coolant system (ECCS) evaluation models submitted by the UPI plant licensees.

In preparing flow instrur'ents in time for the UPTF outage in March-April, 1988, ORNL has completed eight drag bodies and nine breakthrough detectors.

Work is continuing to complete the remaining seven drag bodies and one breakthrough detector. Necessary spare parts and tools are being prepared.

Human Factors Research Plan RES and NRR will brief the Comission on February 2,1988, on the status and i plans for the Agency's human factors (HF) activities. DRPS is coordinating i their HF presentation with that of DLPQE and will provide briefing slides to the EDO by January 27th.

IPE Meetings Two meetings were held with representatives of NRR. The first meeting was held on 1/7/88 between B. Sheron and F. Gillespie and their staff to discuss the role of accident management in the IPE and coordination of other accident management efforts between NRR and RES/DRPS. NRR is also drafting a Commission paper describing how accident management will be implemented.

The second meeting was held on 1/11/88 between RES and NRR to discuss ,

positions regarding the IDCOR Individual Plant Examinatioh Methodology for BWRs.

3 Sr. Management Council Meeting on IPE External Events A meeting was held by the Sr. Management Council for seismic events on January 12th to discuss seismic issues and the Individual Plant Examination (IPE). The Council (consisting of B. Sheron, G. Arlotto. L. Shao, and T. Novak) is charged with the responsibility of providing guidance on the integration of the seismic part of the IPE with other NRC seismic programs.

The next meeting will take place in two weeks to continue the discussion.

Mr. Shao will be coordinating the next meeting.

Human Factors - Cognitive Modeling RES is currently sponsoring work with the Westinghouse Research and Development Center on an artificial intelligence based simulation for analyzing cognitive (intention formation) aspects of NPP personnel behavior. '

FY 1988 work involves operability tests of a Cognitive Environment Simulation (CES) developed earlier in the research program. Recently, AE00 l expressed an interest in using CES to better understand the potretial for l cognitive errors in several NPP events deemed critical to safety. l Preparations are being made with AEOD to carry out these analyses, with AEOD funding, as part of the larger operability tests of CES.

Human Factors - Data Management RES is currently sponsoring work with INEL EG&G to establish a Nuclear Computerized Library for Assessing Reactor Reliability (NUCLARR) deta

, management system. NUCLARR is designed to provide a broad range of human error and hardware failure rate data as inputs to reliability and risk studies. During the week nine NRC staff members representing three offices were trained on NUCLAAR and provided access tc the data bank from their own computer work stations.

Items of Interest Office for Analysis and Evaluation of Operational Data Week Ending January 15, 1988 Incident Response On January 13, IRB staff nembers net with representatives of Department of Energy (DOE), Environmental Protection Agency (EPA), and the Federal Emergency Management Agency (FEMA) in an Interegency Working Group Pe.eting to develop the outline for an Advanced Radiological Assessment course to be given at Emitsburg, Maryland during the sumer. The new course will focus on recovery and reentry issues.

On January 13, the AEOD Deputy Director and an IRB staff nember met with representatives of Office of Governmental and Public Affairs, DOE, EPA, and FEMA to discuss issues related to public infomation during an emergency at the upcoming Nuclear Energy Agency n eting in Paris in February 1988.

On January 14, the AEOD Director and IRB staff members met with representatives of DOE and FEMA to resolve any remaining issues related to the Federal Field Exercise-2 (FFE-2) evaluation report. D e meeting was beneficial and all is m s for the Evaluation Report have been resolved among the Feds.al agencies.

Diagnostic Evaluation and Incident Investigation The McGuire Diagnostic Evaluation Team returned ftp e week at the licensee's corporate offices and plant site on Monday, January 11. The initial drafts for the team's report are scheduled to be completed by January 15. The exit "eeting with the licensee is new scheduled for January 22.

Technical Training The Technical Training Center (TTC) Director, Acting Specialized Technical Training Staff Chief, and an Education and Training Specialist from the Office of Personnel visited the headquarters and local training activities associated with the Chief of Naval Technical Treining, Naval Air Station Memphis, Hillington, Tennessee on January 12-13. The visit allowed firsthand observation and information gathering on a runber of topics involving U.S. Navy technical trainirg programs. Major topics included the use of instructors and educational specialists in technical training development, management, and instruction; the life cycle of technical training courses; different training appraisal techniques; the use of group-aaced and self-paced technical training programs; and the integration of la boratory, simulator, or other hardware training devices with classroom presentations.

Approximately 50 applicants were interviewed by the six-member special OP recruitment team, supplemected by a representative of HMSS. The overall quality of the applicants was high, and the majority of those who successfully passed the typing tes,t will be referred for possible selection threughout the agency. This was the most successful Job Fair the agency has sponsored in some tine; reasons for the high attendance appeared to be the attraction of the White Flint location and the relatively high job security of Federal employment for secretarial and clerical personnel. OP will attend another professionally-sponsored clerical job fair on February 3 at the Sheraton Potomac.

On Tuesday through Thursday, January 12 through 14, 1987, the Division Director and Assistant Director for Projects. TVA Projects. OSP visited the Seguoyah  ;

site to assess the status of Sequoyah, Unit P for heatup and restart feriti- '

cality). TVA is planning an extended heatup of Unit 2 before restart. This is part of TVA's planned activities leading to the restart of Unit 2.

This will be a non-nuclear heatup of both the primary and secondary plant using reactor coolant pump heat. The reector coolant system will be taken to rated temperature and pressure (i.e., 547'F and 2,235 psig). The plant will have ret the operability requirements for Modes 4 and 3 in the Technical Speciff-cations. TVA stated this will be a "shakedown" period for plant equipment, systems and personnel before the reactor is taken critical. The exact duration l

will depend on the nature of the problems encountered and the time needed for operators to refami16rize themselves with an operating plant environment.

1. On January 10-12, Region !! hosted the Utility /NRC Interface !!

meeting in Atlanta. Over 100 utility /NRC people attended the workshop. Region II participation included the Regional Admini-strator, Deputy Regional Administrator, the Directors of the three program Divisions, and 25 staff. All the participants believed the results of the workshop were positive, and many initietives were identified that would enhance comunications and trust.

2. On January 15, 1988, the Deputy Director, Division of Reactor Safety (DRS); a DRS Branch Chief, and a Division of Reactor Projects Branch Chief are in headquarters to attend the NRC/ Alabama Power Company meeting on the unisolatable Safety Injection Pipe Crack at Farley Unit 2. The concerns are threefold: 1) a plant-specific determina-tion of the cause, which appears to be thermal stresses due to cold water leakage past a check va've; 2) assessment of plant specific fixes; and 3) a determination of whether there are any generic ramifications.
3. On January 15, 1980, the Regional Administrator and selected members of the Regional Staff are attending the Thirteenth Annual Labor /

Hanagement/ Government Social Responsibility Awards Breakfast and Seminar at the World Congress Center in Atlanta.

4. An Order is being prepared that would require Georgia Institute of Technology to stop irradiation experinnents until they have fully assessed the causes of a contamination event that occurred August 1987. Sinilar events have also been alleged to occur. On January 7, the licensee was verbally requested to cease irradiation experinents, and the Order will require NRC review of remedial actions prior to authorizing further irradiations. ,

Commissioners Roberts and l Bernthal continue to believe that for purposes of this meeting, attempts to categorize plants are unproductive and should be dropped. Staff should focus its time and efforts on the ,

substance, rather than the categories, of power plant and I materials licensees achievements and problems.

Commissioner Rogers requested the staff to inform the Commission of those plants with silicone-rubber electric cables and the staff's program and schedule to assess the acceptability of its use. l l

Commissioner Rogers also suggested the staff encourage trade societies and associations to provide refrasher courses to materials licensees on the hazards associated with radioactive materials handling.
