RESPONSE TO INSPECTION REPORT 457/87-045 VIOLATION (457/87045-03) 10 CFR, Appendix.B. Criterion V, "Instructions, Procedures, and Drawings," states, "Activities affecting quality shall.be prescribed by -

documented... procedures...of a type appropriate to the circumstances..."

Contrary to the above, the Licensee's onsite review committee reviewed and approved for_use-in_ Unit 2,-six BwEP procedures which directed the Unit 2 operators to use a Unit 1 procedure for actions to take for a safety _ injection (SI) initiation in Unit 2. The referenced Unit 1 procedure is for Unit 1 equipment only. The procedures should have referenced a Unit 2 procedure which directs actions to be ta'oen on Unit 2 equipment only for an SI injection.

RESPONSE

Commonwealth Edison acknowledges that six Unit 2 BwEP procedures were approved which directed the Unit 2 operators to use a Unit 1 procedure.

The safety significance of this occurrence is small due to the fact that because of training given the Unit 2 operator would know to go to the correct Unit 2 procedure, although the Unit 1 procedure was referenced.

Additionally, the procedures in question had never been used.

CORRECTIVE ACTIOF TAKEN AND RESULTS ACHIEVED A temporary procedure change was put in place to direct the Unit 2 operators to use Unit 2 procedures. A permanent procedure revision is being prepared.

CORRECTIVE ACTICNS TAKEN TO AVOID PURTHER OCCURRENCES The Procedure Generation Package (PGP) which controls revising the emergency procedures was revised. To prevent the incorporation of errors into an individual unit's emergency procedures, the PGP now requires that "the verifier will compare the identical Byron /Braidwood Unit I and 2 emergency procedure revisions to ensure consistency of information is maintained" prior to final approval. This revision was put into effect on January 25, 1988.

DATE OF FULL COMPLIANCE The permanent procedure revision is expected to be completed by March 31, 1988.
