The William Powell Company Plant 2 Docket No. 99900057/78-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on July 25-28, 1978, it appears that certain of your activities were not conducted in full accordance with NRC requirements as indicated below:

A. Criterion XII of Appendix B to 10 CFR 50 reavires measures be established to assure that measuring and testing devices used in activities affecting quality are properly controlled, calibrated and adjusted at specified periods to maintain accuracy within necessary limits.

The QA Manual paragraph 6.4.1.1 states in part, "All tools, gages, instruments and other measuring and testing devices, . . . , used in manufacturing affecting quality shall be calibrated a'. specific periods or use intervals to maintain accuracy within necessary limits.

Calibration shall be against certified measurement standards which have known relationship to National standards where such standards exist. Procedures shall be in effect to assure all of these require-ments exist."

C6ntrary to the above, the following conditions existed:

1. The Argon welding gas flow meters in the first floor weld shop and those in the third floor hardfacing department were not calibrated nor was there a procedure in effect to assure that the above calibration requirements were met for these devices.
2. There was no documented evidence available to verify that the calibrated welding generators numbers 5417, 5679, 5752, 5761 and 9149 were calibrated against standards which have known relation-ship to National Standards. Prior to the end of the inspection, the responsible Powell representative secured copies of the subcon-tractor's calibration certificates which the inspector reviewed to verify that the reference standards used in calibrating the above machines, were treaceable to the National Bureau of Standards (NBS).
3. The "Sargent Welch" Standard Thermometer used in calibrating surface Pyrometers Nos. 5905 and 5906, was not calibrated nor was there a procedure in effect to assure that the above calibra-tion requirements were met for this thermometer. Prior to the end of the inspection, the above thermometer was calibrated at Powell Plant No.1 against a standard thermometer, SN434. The inspector verified documented traceability.

4. There was no calibration procedure available for the " Black 1.ight" fluorescent lamp and the ultraviolet calibration reference meter, used in the process of fluorescent dye penetrant inspection.

B. Criterion II of Appendix B to 10 CFR 50 requires the program to pro-vide for indoctrination and training of personnel performia; activities affecting quality.

ASME Code Section III Article NCA-4000, subparagraph 14CA 4134.2.(b) states in part, "The program shall provide for indoctrination and training of personnel performing activities affecting quality as necessary to assure that suitable proficiency is achieved and main-tained."

The William Powell, Quality Assurance Manual Article 8, Paragraph 8.4 states in part, " Quality Assurance Personnel, . . . are trained hy Quality Assurance Management in accordance with a documented training pronram. Records of personnel training are maintained in the Quality Assurance Department."

Contrary to the above, none of the in-process and final dimensional inspectors training sessions were documented.
