NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on August 7-9, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph.NCA-4134.5 of Section III to the ASME Code states in part, " Activities affecting quality shall be prescribed by documented instructions procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings...."
WC-A procedure AWP-24, Revision 5, paragraph 1.6.3 states, " Temperature shall be determined by the use of contact pyrometer."
Contrary to these requirements, the welding interpass temperature was verified by the use of 3500F "Tempelstik" on a four inch (4") N-226SP Gate Valve Body, Purchase Order 30200, Sales Order 7193544.
The quality assurance engineer stopped the welding operation and instructed the welding supervisor that the temperature must be verified b;' the use of the contact pyrometer.
