Appendix A NOTICL OF VIOLATIONS Consolidated Papers, Inc.
License No. 48-01117-01 Based on the inspection conducted on September 12, 1978, it appears that certain of your activities were in noncompliance with NRC require-ments as noted below.
Items 1 through 2 are infractions, and item 3 is a deficiency.
1.
Condition 15. of your license requires that licensed material be possesoed and used in accordance with the statements, re pre sen ta t ions,
and prccedures contained in your letter dated April 21, 1974.
This letter requires that af ter a gauge has been installed a survey will be performed to establish that the level of radiation in the area in which an individual can be is less than 0.5 rem / year.
Contrary to this requirement, no surveys were made after installa-tion of a 25 mci Kr-85 source, Serial No. 06361566, or a 250 mci Kr-85 source, gauge Serial No. TLK2-0313A-128.

Cor>dition 14.A. of your license requires that you test your sealed sources containing byproduct material for leakage and/or contamina-tion at intervals not to exceed six months.
Contrary to this requirement, you failed to leak test a 15 mci Cs-137 source located at the Biron facility between December 30, 1976 and January 30, 1978, a period exceeding six months.
3.
Condition 14.B. of your license requires that records of leak tests be maintained in units of microcuries.
Contrary to this requirement, you failed to maintain the records of your leak tests in units of microcuries.
