Appendix A NOTICL OF VIOLATIONS Consolidated Papers , Inc. License No. 48-01117-01 Based on the inspection conducted on September 12, 1978, it appears that certain of your activities were in noncompliance with NRC require-ments as noted below. Items 1 through 2 are infractions, and item 3 is a deficiency.
- 1. Condition 15. of your license requires that licensed material be possesoed and used in accordance with the statements, re pre sen ta t ions ,
and prccedures contained in your letter dated April 21, 1974.
This letter requires that af ter a gauge has been installed a survey will be performed to establish that the level of radiation in the area in which an individual can be is less than 0.5 rem / year.
Contrary to this requirement, no surveys were made after installa-tion of a 25 mci Kr-85 source, Serial No. 06361566, or a 250 mci Kr-85 source, gauge Serial No. TLK2-0313A-128.
- 2. Cor>dition 14.A. of your license requires that you test your sealed sources containing byproduct material for leakage and/or contamina-tion at intervals not to exceed six months.
Contrary to this requirement, you failed to leak test a 15 mci Cs-137 source located at the Biron facility between December 30, 1976 and January 30, 1978, a period exceeding six months.
- 3. Condition 14.B. of your license requires that records of leak tests l be maintained in units of microcuries.
