Appendix A NOTICE OF VIOLATION St'. Joseph Hospital License No. 12-00610-02 Based on the. inspection conducted on August 23, 1978, it appears that cerealn of your activities were in noncompliance with NRC req:dc.ements, as noted below. The item is an infraction.
10 CFR 20.201(b) requires surveys (evaluations) be made for you to comply with all sections of Part 20.
Contrary to this requirement, no documentation wac available to demonstrate that an adequate evaluation was made of an individual's whole body film badge results to determine compliance with 10 CFR 20.101(a). This individual is a patient transport carrier and the reported film badge results indicated 1500 mrem during the first quarter 1976.
