SUBJECT: NRC INSPECTION REPORT 50-482/96-24 AND NOTICE OF VIOLATION

Dear Mr. Carns:

'

An NRC inspection was conducted December 1,1996, through January 11,1997, at your

Wolf Creek Generating Station reactor facility. The enclosed report presents the scope and

results of that inspection.

,

During this inspection, three violations rvere identified in the operations, maintenance, and

engineering areas. The first violation involved an operability evaluation of moisture

intrusion into the turbine-driven auxiliary feedwater pump governor oil you made without

acknowledging or evaluating vendor requirements for the equipment. This is of particular

concern because your initial plans were to operate with an elevated moisture content for

approximately 6 weeks, in part due to maintenance rule considerations, suggesting a

continued lack of rigor in establishing the technical requirements for this very important

i

safety component. The second violation involved maintenance workers failing to follow

~

your procedures in not installing the spindle nut cotter pin in one of your main steam safety

,

valves. This violation is of concern because of the potential for the missing cotter pin to

i

result in the f ailure of the valve to reseat. While you identified this violation and took

appropriate immediate corrective actions, this item is being cited because of the potential

j

,

significance of this type of error. Your response to this violation should clearly describe

'

your corrective actions to prevent recurrence. The third violation involved your failure to

accurately document the basis for why a change to your Updated Safety Analysis Report

did not involve an unreviewed safety question. This violation is of concern because it

i

suggested a lack of rigor in preparing the safety evaluation.

These violations are cited in the enclosed Notice of Violation (Notice) and the

circumstances surrounding the violations are described in detail in the ecciosed report.

Please note that you are required to respond to this letter and should 'ollov/ the

instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary

to ensure compliance with regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).

To the extent possible, your response should not include any personal privacy, proprietary,

or safeguards information so that it can be placed in the PDR without redaction.
