Dear Mr. Denton:

This report is submitted in accordance with the requirements of Appendix B to Operating License DPR-49, Specification 3.3.1.C.5.

Problem on September 11, 1978 it was determined that the monthly functional I

surveillance test of the reactor building stack radiation monitor l

was not performed in July.

The surveillance test was to be done l

on July 26, 1978 and was not done until the suisequent test on August 24, 1978.

Investigation The surveillance test was not done due to an apparent routing error.

The test is normally distributed by the surveillance coordinator to the radiation protection department and then routed to the electrical maintenance department which actually performs the test.

For some unknown reason the test was not in the electrical maintenance surveillance test file until the August tests were issued.

This same problem was identified in ETSV Report 78-6.

Corrective Action In order to help preclude routing errors, the surveillance test will be issued directly to the electrical maintenance department.

In addition, the test will be listed on the electrical maintenance monthly surveillance test issuance list.
