NRC INSPECTION REPORT NO. 50-440/96-11 AND

NOTICE OF VIOLATION

Dear Mr. Myers:

On November 1,1996, the NRC corapleted a routine safety inspection at your

Perry 1 reactor facility. The enclosed report presents the results of that.

inspection.

During the 7-week period covered by this inspection period, your conduct of

activities at the Perry facility was generally characterized by safety-

!

conscious operation, appropriate maintenance practices, and sound engineering

'

practices.

The facility was operated safely. One plant event, involving the

trip of a motor control center feeder breaker, occurred that impacted plant

o]eration, but did not cause a plant transient.

Your staff responded well to

.

t1is event. However our evaluation of the equipment malfunction that

contributed to this event has not been completed.

This item is of concern

because it had the potential to cause a loss of safety function of the control

room emergency recirculation system.

A violation of NRC requirements was identified.

In this case, your personnel

i

identified that a change to an instruction had made the instruction unusable

l

as written. One of your unlicensed operators identified the error while NRC

inspectors were observing his attem)t to follow the instruction.

Your

Jersonnel further identified that t1e erroneous section of the instruction had

,

Jeen completed several times without the error being identified. .The delay in

,

identifying the error concerns us, as does your failure to identify the error

!

during review of the instruction prior to use.

This violation is cited in the enclosed Notice of Violation, and the

circumstances surrounding the violation are described in detail in the

'

enclosed report.

Please note that you are required to respond to this letter

,

and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to

,

determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.
