Violation 50-424/85-60-01 and 50-425/85-40-01, " Failure to Comply With Welding Requirements" - Severity Level V l

The violation identified the following specific discrepancies:

a.

Fabrication drawing M01G-S400 (Revision 1) contained an inappro-priate weld symbol at coordinate E7.

b.

The Georgia Power Company quality control inspector accepted the completed weld even though it did not comply with the erroneous symbol on drawing M01G-S400.

c.

A welder was prepared to perfors a weld on pipe support V2-122b-174-H642 using 3/32 inch diameter welding filler material with a welding machine set at 35 amps.

Pullman Power Products (PPP) welding procedure specification (WPS) 27-III/I-8-08 (Revision 4/14/83), which is applicable for this work, requires a welding current range of 100 to 150 l

amps.

Georgia Power Company. offers the following response pursuant to the criteria of 10 CFR 2.201:

Page Two While drawing M01G-S400 did reflect an inappropriate weld symbol at coordinate E7, the typical detail drawings and notes provide direction for the resolution of this type discrepancy.

Specific details which are applicable to the Pc.A to Pc.C connection in the containment stack support platform shown on drawing M01G-S400 are contained in typical detail drawings AX2D94V019 (Revision 32) and AX2094V002 (Revision 14).

General Notes drawing AX2094V019 (Note 3G under " Structural Steel Connections") states:

"All flare-bevel welds for tube sections shall be done in accordance with Detail 34 on drawing AX2D94V002."

Detail 34 on drawing AX2D94V002 shows the correct flare-bevel weld detail that was used in the fabrication of the cited connection and in the subsequent inspection by QC.

However, only drawing M01G-S400 was referenced on the QC inspection report.

In the USNRC inspection report it was noted that the GPC QC inspector stated that there was no documentation to support the " designer's inten-tions".

The QC inspector's statement was incorrect.

As noted above, the designer's intentions are documented in notes and details on the typical detail drawings.

Georgia Power Company QC inspection personnel have the responsibility to inspect to known standards and controlled design documents.

Inspectors have been instructed to reference on inspection reports all documents used in their inspections.

The USNRC inspection report cites PPP WPS 27-III/I-8-08-12 (Revision 4/14/83) as requiring a welding current range of 100 to 150 amps when using 3/32 inch diameter filler material. The WPS actually allows various sizes of weld filler material to be used in any one of three current ranges (50-100, 100-150, or 150-200) depending on the voltage and travel speed ranges.

The WPS states, "Parsmeters for 1/16", 3/32", or 1/8" must fall within one of these three ranges."

l The current settings on welding nachines do not accurately reflect the amperage available at the welding operation.

Pullman Power Products personnel regularly check welding machine current outputs in accordance with procedure X-10 to ensure compliance with WPS requirements and to familiarize welders with machine output correlations.

The welding machine being used to make the weld on pipe support V2-1228-174-H642 was actually set at 37.5 amps (35 amp setting on the rheostat control plus 2.5 amps on the toggle switch).

The current output of this machine was checked by a PPP welding coordinator on December 20, 1985, and the machine was found to provide an output of 54 amps at the torch when set at 37.5 amps.

Therefore the welder was operatirg within one of the current ranges allowed by WPS 27-III/I-8-0B-12.

Our investigation into the circumstances surrounding the violation shows that the welder observed by the NRC inspector was preparing to perform a

tacking operation on a

shim plate for pipe support V2-1228-174-H642.

Reduced welding machine current settings are appropriate for this type of operation.

=e Page Three Georgia Power Company quality assurance will conduct a comprehensive ASME/AWS assessment audit during March 1986.

The audit will evaluate the adequacy of PPP welding procedures and will ensure that adequate controls are in place to assure compliance with welding procedure para-meters. The audit is expected to be completed by April 1,1986.
