Your nuality Assurance Progran Approval for Radioactive Material Packages No. 0421 expires on April 30, 1996.
l If you are an NDC licensee and conduct activities under the General l Licenses of Subpart C of 10 CFR Part 71 or an Agrecrent State licensee subject to the requirerents of 10 CFR Part 71, as renuired by 10 CFP.
Part 150.20, a cuality assurance program apprnved by the Cornission as satisfying the provisions of Subpart H of 10 CFP. Part 71 is required.
You should request renewal of your Ouality Assurance (0A) progran at least 30 days prior to the expiration date. This will provide for continuation of your QA program to satisfy part of the provisions of Subpart C of 10 CFP Part 71 until a final detennination has been taken on your application.
Unless an exemption is provided by 10 CFR Part 170.11, an application fee of $150.00, as required by 10 CFR Part 170.31, should be included j with your request for renewal. This notice of expiration should not be l construed that such notice will be provided in the future. If you do not desire to renew your OA program, please let me know.
