If you are an !!RC lic?nsee and conduct activities under the General Licenses of Subpart C of 10 CFR Part 71 or an Agreement State licensee subject to the recuirenents of 10 CFR Part 71, as required by 10 CFR Part 150.20, a quality assurance progran approved by the Connission as satisfying the provisions of Subpart H of 10 CFR Part 71 is required.

You should request renewal of your Quality Assurance (OA) program at least 30 days prior to the expiration date. This will provide for l

continuation of your QA progran tc satisfy part of the provisions of Subpart C of 10 CFR Part 71 until a final detemination has been taken on your application.

Unless an exemption is provided by 10 CFR Part 170.11, an application

fee of $150.00, as required by 10 CFR Part 170.31, should he included with your request for renewal . This notice of expiration should not be j construed that such notice will be provided in the future. If you do <
