NOTICE OF VIOLATION DATED APRIL 25,1997
Dear Mr. Marcantel:
This acknowledges receipt of your letter dated May 13,1997, in response to our letter dated April 25,1997, transmitting a Notice of Violation.
The violation cited in our April 25,1997 letter is considered repetitive because it reasonably could have been prevented by the corrective actions specified in your letter dated March 5,1996. However, the dissimilarity of the root causes of the violations identified during our inspections on March 20,1996 and April 15,1997, the adequacy of your previous corrective actions, and the number of times the violation identified on April 15,1997, occurred were considered in assessing the severity level of the violation cited in our letter dated April 15,1997.
We have reviewed your corrective actions, which appear to be adequate, and have no further questions at this time. These corrective actions will be examined during a future inspection.

NRC INSPECTION AND NOTICE OF VIOLATION REPLY TO NOTICE OF VIOLATION License #1L-01545-01 Docket #999-90003 In answcr to your letter dated 4/25/97 regarding the audit conducted on 4/15/97, NTS Enterprises, inc. does not feel that this sh; I be treated a a repeat item. to the NRC. This consisted of an employee nucarrying out his duties and correction action was taken against said employee.
l In answer to the violation discovered by your auditor on 4/15/97, steps were taken to find out why l
this occurred. The date of said violation was May 16,1996.
STEP #1 Went back to the original call out for work which was scheduled for May 15,1996 at Smith Road, Lemont, Illinois. The front office was not informed of a different jobsite on May 16, 1996 in East Chicago, Indiana. This was i. case of miss communication by the company inspector and NTS Enterprises technician info ming the front office of change in location.
l STEP #2 Correction action taken to avoid this in the future is as follows: A letter was posted above the time clock and the door of our radiation vault, stating to notify the front office on any l
job locat.an changes from the original location on the work order.
l STEP #3 The Radiation Safety Office also called a radiation safety meeting with all technicians i
concerning the importance of the NRC Regulations 10 CFR 150.20(a) and 10 CFR 150.20(b).
Also discussed was the importance of commu1ication with the front office regarding job location change. In addition to the above, the notice regarding location changes shall stay posted.
