SUBJECT: NRC INSPECTION REPORT 50-382/97-05 AND NOTICE OF VIOLATION

Dear Mr. Dugger:
An NRC inspection was conducted April 21-25,1997, at your Waterford Steam Electric
Station, Unit 3 reactor facility, with in-office inspection and followup continuing through

May 1,1997. A supplemental telephonic exit was held on May 1,1997. The enclosed
report presents the scope and results of that inspection.
As a result of our inspection findings, a violation was identified regarding the
implementation of your welding program. We also recognize that you have identified a
i
declining trend in welding activities through your corrective action program. Recent
problems that have been identified include lack of control of weld rod, carbon
contamination of stainless steel welds, missed final quality control inspections of stud
i
!
welds, and welding on a wrong system. In regard to the latter issue, many barriers to
j
failure were breached by craft and quality control personnel. This situation could have
resulted in a hazard or system inoperability had not the fortuitous discovery of the error not
been identified by a new work crew. In the aggregate, these issues are of concern and
indicate that improved welding controls are necessary.

The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding this violation are described in detailin the enclosed report. Please note that

you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,

its enclosures, and your response will be placed in the NRC Public Document Room (PDR).
To the extent possible, your response should not include any personal privacy, proprietary,
or safeguards information so that it can be placed in the PDR without redaction.
