MODIFICATION N0. 1 TO TASK ORDER NO. 5 UNDER CONTRACT NO. NRC-33-93-225 ENTITLED " FORMS AUTOMATION AGENCY IMPLEMENTATION" The purpose of this task order modification is to reflect the change in the work requirement of the subject task order as a result of the' partial termination of work issued by the U.S. Nuclear Regulatory Commission on April 21, 1997. As you are aware, the partial termination was issued due to a delay by the Government in the Agency-wide installation of the software entitled "GroupWise," which is not scheduled to be completed until September 1997.
Enclosed'is a copy of the revised Statement of Work whereby the period of performance has been extended from May 30, 1997 to June 30, 1997, to allow for continued performance of the remaining tasks.
The effective date of this modification is April 21, 1997. All other terms and conditions of this task order remain unchanged, including the ceiling amount of $311,759.29.
However, as reflected in the April 21, 1997, letter, an equitable adjustment, if any, to the task order ceiling amount will be made at a later date due to additional tasks that will be added to this task order shortly.
Acceptance of this modification should be made by executing three copies of this document in the space provided and returning two copies to Ms. Brenda J.
DuBose, Contract Specialist. The third copy may be retained for your records.
