1 of 1 MONTHLY OPERATING ACTIVITIES - JANUARY, 1986 HIGHLIGHTS:

The reporting period began and ended with the reactor in Mode 1 at 90% rated thermal power.

Three Engineered Safety Features (ESF) actuations occurred during the reporting period.

Gross electrical generation from the month of January was 707,310 MWH.

SUMMARY

01-07-86 At 0452, No. 1 Boric Acid Transfer Pump was declared inoperable due to a pump leak.

The pump was declared operable at 1847 on 01-16-86.

01-20-86 At 0449, an Engineered Safety Features (ESP) actuation occurred due to a high alarm on containment gaseous radioactivity monitor ERS-1305, resulting in a containme'nt purge isolation signal.

The increase in containment activity is believed to be due to packing leakage while cycling PORV motor operated isolation valve, 1-NMO-151, as part of IST valve operability tests.

At 1439 and 1440, Engineered Safety Features (ESP) actuations occurred due to high alarms on containment particulate radioactivity monitors ERS-1301 and ERS-1401 respectively, resulting in containment purge isolation signals.

01-31-86 At 1155, and earthquake occurring 30 miles northeast of Cleveland, Ohio, measuring 5.0 on the Richter scale.

No observable effects were sensed in the plant and no seismic instrumentation was activated.

A RCS leak rate test was initiated as well as a tour of all plant safety related areas.

No increase in RCS leakage er damage was found.

MAJOR SAFETY-RELATED MAINTENANCE JANUARY, 1986 M-1 Discovered packing leak and body-to-bonnet leak on 1-IRV-300 RHR Pump Discharge Line Regulating Valve to CVCS Purification System. Disassembled and cleaned valve.

Reassembled with new gaskets and packing.

Retested valve satisfactorily.

l M-2 2-CS-414-3, Primary Water Valve to No. 3 Boric Acid Transfer j

Pump suction was leaking by.

Replaced diaphragm and gasket.

Valve now works properly.

l M-3 Unit 1 No. 1 Boric Acid Transfer Pump was leaking. Rebuilt pump, reinstalled, and retested satisfactorily.

I C&I-1 Bistable 2PB-456D, Pressurizer Pressure Set II, drifted twice

}

in two weeks.

Replaced defective bistable with new bistable, performed calibration, performed surveillance test procedure and declared Pressurizer Pressure Channel Set II operable.
