NOTICE OF VIOLATION Comonwealth Edison Company Docket No. 50-455 As a result of the inspection conducted on December 2-6, 1985, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
10 CFR 50, Appendix B, Criterion XI as implemented by the Comonwealth Edison Quality Assurance Manual, Quality Requirement 11.0, and the Byron Startup Manual requires that a test program be established. This test program is required to demonstrate that systems and components will perform satisfactorily in service by performance of written test procedures which incorporate the requirements and acceptance limits contained in applicable design documents.
Contrary to the above, Unit 2 preoperational test SI 73.62, " Safety Injection
- Flow Balance," was written and approved with inappropriate acceptance criteria in that the test required the centrifugal charging, safety injection, and residual heat removal pumps to equal or exceed the head versus flow performance curves from the vendor's shop tests. (455/85045-04)
This is a Severity Level IV violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
