SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-334/97-04, 50-412/97-04

NOTICE OF VIOLATION

Dear Mr. Cross: l

The NRC conducted an inspection of the Beaver Valley Power Station (BVPS) facilities from

April 27 to June 7,1997. The enclosed report presents the results of that inspection.

During the 6-week period covered by this inspection period, your conduct of activities at

the Beaver Valley Power Station facilities was generally characterized by safe operation

and maintenance of station equipment. Recent revisions to your Maintenance Work

Request process and Fix-It-Now maintenance program have streamlined work processes

and have helped to reduce the non-outage corrective maintenance backlog. Actions to

evaluate and correct an inoperable station battery cell were timely. Notwithstanding,

certain performance weaknesses were evident and warrant continued management

attention. Specifically, Unit 1 operator response to inoperable analog rod position

indications revealed weaknesses in operator use of technical specifications, operating crew

communications, shift turnover review, and logkeeping practices. Also, work activities on

the service water and auxiliary feedwater systems were not effectively coordinated to

minimize safety related equipment out of service time.

The security program was inspected during this period with a special focus of security l

equipment condition and rnaintenance, search and admittance control, and land vehicle

control measures. These areas were effectively implemented and actively supported by

management. The inspectors determined that you are implementing a security program

that effectively protects public health and safety.

You have established a generally effective program for the processing of liquid and solid

radioactive wastes and the transportation of radioactive materials, in severalinstances,

our inspectors found that your procedures and related documents were minimal relative to

content, process description, definition, guidance or instruction.

Based on the results of this inspection, the NRC has determined that a violation of NRC

requirements occurred. This violation is cited in the enclosed Notice of Violation, and the

circumstances surrounding the violation are described in detailin the enclosed report. This

violation is of concern because it indicates that operators need to be more attentive to

plant conditions and the thoroughness of shift turnovers. Please note that you are requirec!
