APPENDIX A NOTICE OF VIOLATION Louisiana Power & Light Company Docket No.:
50-382 Waterford Steam Electric Station, Unit 3 License No.: NPF-26 During an NRC inspection conducted on December 2-6, 1985, a violation of NRC requirements was identified. The violation involved the failure to adequately evaluate and report a defect to the NRC.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions " 10 CFR Part 2 Appendix C (1985), the violation is listed below:
Failure to Adequately Evaluate and Report a Defect in General Electric (GE)
AKR-50 Circuit Breakers Safety-Related Basic Components 10 CFR Part 21 requires the licensee to perform evaluations of deviations and to report defects in basic components which could create a substantial safety hazard.
Contrary to the above, the licensee failed to adequately evaluate and report to the NRC the failure of a GE Type AKR-50 circuit breaker, which rendered the B charging pump inoperable.
The defect, identified by the GE evaluation and engineering analysis performed for the licensee, is generic for both GE Type AKR-50 and AKR-30 breakers.
The defect could create a substantial safety hazard.
This is a Severity Level IV violation (Supplement I D.) (382/8531-03).
Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
