IMMEDIATE ACTIONS TAKEN FOLLOWING Tile IIIGII RADIATION AREA ACCESS EVENT ON MAY 1,1997 A.

SUMMARY

OF EVENT On May 1,1997, at approximately 3:00 a.m., a Baltimore Gas and Electric Company (BGE) electrician entered the Unit 2 Containment, without the required dosimetry, to commence work on reconnecting No. 21 A Reactor Coolant Pump (RCP) motor. The electrician had entered the Radiologically Controlled Area (RCA) with the required dosimetry, but had left it at the dress-out area when putting on his required anti-contamination clothing. Number 21 A RCP bay was e posted High Radiation Area (HRA). The area in which the electrician was working to reconnect the motor was a posted low dose area within No. 21 A RCP bay. The electrician was assigned an estimated dose of 10 mrem based upon his stay time in the work area and the general work area dose rates (4-6 mrem / hour) at No. 21 A RCP motor. Another BGE employee discovered the electrician's missing dosimevy in the dress-out area and reported this to Radiation Safety personnel. The electrician exited Unit 2 Containment at approximately 4:25 a.m. when summoned by Radiation Safety personnel. Neither the contractor health physics technician at the Containment Air Lock access point nor the contractor health physics technician that provided coverage during the work at No. 21 A RCP motor, noticed that the electrician was not wearing the required dosimetry.

IMMEDIATE ACTIONS TAKEN o

Electrician (BGE employee) was removed from the.RCA.

o Electrician's access to the RCA was suspended and a dose assessment of the event was initiated.

o An Issue Report was initiated documenting the event and recommending actions to resolve the issue.

o Plant General Manager stopped all work in llRAs. Plant General Manager authorization required to resume work in liras.

o Site personnel notified of the IIRA work stoppage via the plant paging system.

o Preliminary interviews were conducted with personnel directly involved in the event.

o Commenced a lluman Performance Enhancement System evaluation root cause analysis.

o Nuclear Regulatory Commission Resident Inspectors were briefed on the event.

o Verified that the electrician was properly logged in under the correct Special Work Permit.

A training record review verified that the worker was qualified to perform work in the llRA.

o Commenced development of procedural access verification checklists to be used for personnel entries into RCAs, Containment, and liras.

increased Radiation Safety supervisory oversight coverage during non-normal working hours.

~ INTERIM CORRECTIVE ACTIONS TAKEN PRIOR TO RESUMING WORK IN HIGH RADIATION AREAS

Additional controls were added to an established plant procedure - (Radiation Safety Procedure 1-104). The controls consisted of the implementation of checklists for routine access verification prior to personnel entry into Radiologically Controlled Areas. Containment, and High Radiatior,~ Areas. (see Attachments (3)and(4)]

Appropriate Radiation Safety and contractor personne! were tramed on the procedural changes (i.e., checklist requirements for access verification) to Radiatica Safety Procedure 1-104.

In addition, increased Radiation Safety supervisory tours are currently being conducted in the field i

to ensure proper imp!cmentation of the new checklist requirements discussed above.

The Plant Ger.cral Manager authorized the resumption of work in High Radiation Areas at

- i approximatdy 8:45 p.m. on May 1,1997.

The P' ant General Manager issued a memorandum (dated May 1,1997), with the access verification checklists attached, to all Calvert Clifti, Site Supervisors directing them to promptly discuss these checklists with their personnel to ensure ti ey understand and comply with the new requirements for working in Radiologically Controlled Areas.
