This letter is w fulfill the 14 day report requirements of sections 6.5.2 and 6.6.2 of the Penn State Breazcale Reactor Technical Specifications (TS), for our reportable occurrence of May 1,1997.

The event was initially reported by telephone and fax to Mr. Tom Dragoun of Region I and Mr.

Marvin Mendonca ofIIcadquarters on May 2,1997.

On May 1,1997 during the conduct of SOP-2, Reactor Checkout, step M-6 which collects PSBR '

water data was not completed. This was discovered on May 2,1997 during the conduct of SOP-2.

Tech Spec 4.3.3 requires pool water conductivity to be measured and recorded daily when the reactor is to be operated. During May 1,1997 the reactor was operated. Information exists which -

demonstrates the coolant conductivity limits of Tech Spec 3.3.5 were met at all times.

Interviews were conducted with the duty SRO on May 5 and the checkout RO on May 6. The SRO and subsequent operators of the day of the event operated the reactor on the premise that the

, log book checkout stamp indicated completion of the checkout. The RO was aware of the  !

requirement for the conductivity of the pool water to be taken each day on which the reactor is 1 c

operated (and documented on the pool water data sheet). The root cause of the event is operator i

inattention to detail. Contributing factors may have been that the RO checked that the needed swipe data was present (normally from the preceding day) and then checked if the requimd pool water data was present (which is su pposed to be taken as part of the daily checkout). He indicated he 4

saw the data had been taken ne previous day, but he made a significant error in judgment since it had not been taken for the checkout which he was performing. Another factor is that he incorrectly 1

assumed another facility individual who frequently takes 'he pool water data had probably done so.

Corrective action involved counseling of all licensed staff on the necessity of taking water data to '

, fulfill the Tech Spec requirement to record conductivity daily when the reactor is operated. More detailed and focused counseling was given to the RO responsible for the event pertaining to his

. responsibilities.

L An improvement was made to the pmcedure to emphasize that the water data must be for the current day. During the investigation it was also discovered that since the pool recirculation system  :

i recirculation system would not have been available. The procedure now directs that if the recirculation system conductivity cell cading is not available, a grab sample of pool water will be l taken for laboratory analysis for conductivity before the checkout is completed.
