Program Approval for Radioactive Materials Packages Approval Nunber 0529, Rev. 2 This is in regards to the operations of Therapy Services Incorporated as licensed by the State of Maryland License No, MD-21-009-01, and performed under the Q. A. shipping program, approval number 0529, Rev.
2, as authorized by the U. S. Nuclear Regulatory Comission.
Therapy Services Incorporated has ceased all licensed activities, operations, and radioactive shipments. We have requested that the State of Maryland tenninate our R. A. M. license as of July 31, 1997.
l We are requesting termination of NRC program approval of our Q.' A.
shipping program, and any general operations that we current 1sj fund by payment of an annual fee as of July 31, 1997. The fee has been paid for the upcoming year. Please notify the proper departments so that i
no further invoices will be issued to Therapy Services from the NRC.
