On May 10,1997, at 2005 hours0.0232 days <br />0.557 hours <br />0.00332 weeks <br />7.629025e-4 months <br />, UF detection smokeheads.were deactivated on cell 33-6-6 in 6

the X-333 Process Building while the cell was operating above atmospheric pressure. The

smokeheads in cell 31-3-6 in X-330.

Cell 31-3-6 was offstream for maintenance activities. The CADP system was reporting false alarms from cell 31-3-6, which could be due to dust from maintenance activities or temperature

changes due to the cell being out of service. Since they were not needed to monitor an operating i

cell, the smokeheads for cell 31-3 6 were declared inoperable on May 19,1997, at' 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, but were not immediately deactivated.

j Following shift change, an operator was assigned to deactivate the smokeheads. When the operator accessed the CADP terminal system transients were experienced. On the day of this l

incident the plantsite experienced a lightning storm, which may have contribute <l to the system transients. The operator reported that the CADP terminal was blinking on and off. When the -

operator attempted to enter the required data the system was unresponsive. The operator j

attempted to enter the data 5 or 6 times before the entry was successful.

When the operator was successful in accessing the program he typed ".CELLOF". When the i

- CADP program prompted, " ENTER UNIT NAME WHICH CONTAINS THE CELL YOU L

WANT TO ACTIVATE OR DEACTIVATE", the operator entered "X-33-6" instead cf "X-31-3". The operator then entered the cell number "6" and the command to deactivate "ALL" i

smokeheads.

The operator immediately saw that the appropriate lights on the mimic panel had not gone out.

The operator discovered he had entered the wrong cell number and contacted his supervision and the X-333 Building Area control Room (ACR) #1 Operations phrsonnel in ACR-1 had already responded to the loss of the smokeheads and were establishing a smoke atch, as required by the Technical Safety Requirement (TSR), while investigating the cause of tu incident. On May 19, 1997, at 2017 hours0.0233 days <br />0.56 hours <br />0.00333 weeks <br />7.674685e-4 months <br /> the operator re-activated the cell 33-6-6 smoke detectors. Cell 33-6-6 was operated for approximately 12 minutes without CADP coverage.

According to TSR 2.2.3.3 for the Portsmouth Gaseous Diffusion Plant,50% of the CADP smoke detectors are required to be operable when the cell is operating above atmospheric pressure. In this instance, cell 33-6-6 was operating above atmospheric pressure, which would make the loss of all cell smoke detectors reportable in accordance with the 10CFR 76.120 (c) (2).

Initially the Plant Shift Superintendent determined the event was not reportable because the error i

was immediately detected and all TSR requirements for loss of smoke.tector coverage were met. On May 21,1997, after consultation with Nuclear Regulatory Affairs, it was determined the briefloss of smoke detector coverage met the reporting criteria of 10CFR 76.120 (c) (2). The NRC Operations Center was notified of the event on May 21,1997 at 1029 hours0.0119 days <br />0.286 hours <br />0.0017 weeks <br />3.915345e-4 months <br />.

There was no failed equipment or release of hazardous material associated with this event.

Cause of Event The cause for the loss of CADP smokehead coverage of cell 33-6-6, while it was operating above atmospheric pressure, was an operator entry error. The operator entered an incorrect unit designation for the smoke detectors which were to be taken out of service. The operator obtained proper approvals to deactivate the cell 31-3-6 smokeheads. He accessed the ".CELLOF" function. The operator entered unit "X-33-6" instead of"X-31-3". The operator then entered the cell number "6" and the command to deactivate "ALL" smokeheads. An incorrect entry deactivated all the smokeheads in the wrong cell (33-6-6).

Management has initiated the STAR (Stop, Think, Act, Review) program training with the goal

. of preventing occurrences like this through self-checking. Full implementation of this philosophy would have reduced the likelihood that this event would have occurred. In addition, preventing the use of the ".CELLOF" function for deactivating the smokeheads will improve opportunities for operator self-checking before more than 50% of the smokeheads in a cell become deactivated.

Extent of Exposure ofIndividuals to Radiation or Radioactive Materials There were no exposures to individuals from this incident to radiation or radioactive materials.

Lessons Learned Operators need to use the principles of the STAR program to properly self-check their work.
