1,1995 and is required to be tested at intervals not to exceed six months" The reason for this violation is that the RSO failed to remember to perform these tests.

The corrective steps taken are:

1)The source has been tested and the area surrounding the source hr.s been monitored. The results, negative, are enclosed.

2)A tickle file has been instituted with the secretary and with the RSO for the purpose of reminding the RSO to perform this test every four months. These times will be at the beginaing of the Fall and Spring terms and the beginning of the summer sessions.

3)The University will now be informed by Applied Health Physics, Inc.

when a leak test is due.

Concerning violation B 1"A routine survey has not been performed since Nov.1,1995.

The reason for this violation is that the RSO failed to remember to perform these tests.

The corrective steps taken are:

1)the routine survey was accomplished at the same time as the wipe l

test and the results have been logged and found to be negative.

2)A tickle file he t>cen instituted with the secretary and with the RSO for the purpose of reminding the RSO to perform this test every four months. These times will be at the begir'ning of the Fall and Spring terms and the beginning of the summer sessions.

Concerning violation B-2: "The Ludlum Model 15 (Portable Neutron Survey Meter) is not being l

commercially calibrated annually.

The reason for this violation is that the University has been allemating calibrating the Ludlum Model 15 with its Ludlum model 3 survey meter. A copy of the calibration certificate for the model 3 survey meteris enclosed. At the time of the inspection the Ludlum model 3 survey meter and its calibration certificate were not found and could not be presented to the inspectors.

Corrective Steps taken:

l 1)The Model 15 meter is being sent out for calibration immediately.

On another note, during the inspection it was recommended that the door to the room in which the howitzer is stored, be r@ laced with a more secure door. ESU has replaced that door with a steel door with tamper proof hinges.

As a final comment please know the University is on the DOE list for disposal of this source and is j
