NOTICE OF VIOLATION Dairyland Power Cooperative Docket No. 50-409^'
As a result of.the inspection conducted on March 10-14, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified:
Technical Specification 6.11 requires that procedures for personnel radiation protection be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained, and adhered to.
Procedure HSP-02.6,-Section 5.9, requires that beta dose equivalent rates be determined along with gamma dose equivalent rates if piping or components of systems are opened, drained, or in areas where surface contamination in excess of 500,000 dpm/ft2 smearable is evident or expected.
Contrary to the above, beta dose equivalent rates were not determined or known while workers perfor.med tasks in the reactor head cavity area where smearable contamination levels on the cavity walls were up to 7,000,000 dpm/ftz, This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
