Dear Mr. Jankovich,
Nuclear Containers, Inc. (NCI) requests that its Quality Assurance Program Approval No. 0179, Revision 5, as well as the QA Program itself, be amended to show Eco-Pak Specialty Packaging (ESP), a division of The Columbiana Boiler Company (CBC), as the holder of this QA ?rogram.
CBC has purchased the assets of NCI and has established ESP as a new division of CB ' in its g
place. Therefore, the name Nuclear Containers, Inc. (NCl) will be replaced with Eco-Pak Specialty Packaging (ESP), a division of The Columbiana Boiler Company throughout the body of the QA Program. The technical content of the QA Program will not change.
k ESP will be responsible for maintenance of the QA Program upon implementation and the QA Program Approval Certificate.
Please find enclosed the amendment fee of $250.00.
