SUBJECT: NRC INSPECTION REPORT 50-482/97-08 AND NOTICE OF VIOLATION
Dear Mr. Maynard:
An NRC inspection was conducted February 23 through April 5,1997, at your Wolf Creek
Generating Station reactor facility. The enclosed report presents the scope and results of
that inspection.
During this inspection, four violations were identified in the operations, maintenance, and
engineering areas. Violation A is of particular concern because a supervisor directed
radwaste operators to exceed a precautionary filllimit without L oowing the proper plant
administrative procedures to obtain authorization. Violation B is also of concern because it
is another example of corrective actions to identified problems not preventing the
recurrence of related events. Violation C addresses the lack of rigor and the procedural
noncompliance that your staff used as they considered risk while adding activities into your

work schedule. Violation D cites your failure to update the Updated Safety Analysis Report
to address the configuration of safety-related systems.
These violations are cited in the enclosed Notice of Violation (Notice) and the
circumstances surrounding the violations are described in detailin the enclosed report.
Please note that you are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your response. The NRC will

use your response, in part, to determine whether further enforcement action is necessary
to ensure compliance with regulatory requirements.
We also identified an additional example of a violation cited in NRC Inspection Report 50-
482/97-04 (Violation A). This example reinforces our concern that shift supervisors may
not be consistently recognizing the impact of maintenance activities on the operability of
safety-related equipment. In your response to this previously cited violation, please include
a discussion of this additional example and how your corrective actions will prevent its
recurrence. In order to provide you ample time to include this additional information in
your response, we have authorized an additional 14 days for response to the violations
cited in NRC Inspection Report 50-482/97-04.
