SUBJECT: NRC INSPECTION REPORT 50-416/97-03 AND NOTICE OF VIOLATION

An NRC inspection was conducted February 9 through March 22,1997, at your Grand
,
,
l
Gulf Nuclear Station reactor facility. The enclosed report presents the scope and results of
'
that inspection.
During the 6-week period covered by this inspection period, your conduct of activities at
the Grand Gulf facility was generally characterized by safety-conscious operations, sound
engineering and maintenance practices, and careful radiological work controls. We are
concerned, however, about the violations of NRC requirements that were identified. The
first violation is of concern because a design engineer, with the knowledge of his
supervisor, delayed issuance of a condition report on a nonconforming condition for nearly
4 days because of an impending long weekend. This denied licensed operators the
opportunity to determine operability and assess the impact on the operating license in a
timely manner.
The second violation concerns health physics technicians not documenting a radiological
survey in accordance with procedures and an area radiological survey map used to inform
radiological workers of present radiological conditions not being properly updated. This is
of concern because it is the second time that NRC inspectors have brought to your

attention the fact that area radiological survey maps were not being properly updated.

The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter,
its enclosures, and your response will be placed in the NRC Public Document Room (PDR).
