SUPPLEMENT TO CONFlRMATORY ACTION LETTER Rill-96-OO8 I
Dear Mr. Subalusky:
On July 2,1996, Confirmatory Action Letter (CAL) Rill-96-OO8 was issued to Mr. R. E.
Querio by this office due to events at LaSalle County Station involving the injection of expandable grout material into cracks in the lake screen house floor, creating t':e resultant potential that this material could enter the safety and non-safety related service water I
systems affecting their ability to function. Since the service water event, a number of i
activities have occurred at LaSalle County Station. On September 20,1996, Unit 2 was shut down for a refueling outage. On September 22,1996, Unit 1 was thut down to i
repair a failed turbine control valve. During an NRC service water system operational performance inspection completed on September 24, an operability concern with the i
residual heat removal service water heat exchanger was identified. As a result, Unit 1 was placed in cold shutdown on September 26 pending resolution of this issue. Subsequently, you decided not to restart either unit until the material condition, human performance, corrective action process, and engineering support and design deficiencies, identified by the NRC, your staff, and an independent Safety Assessment, had been resolved. This supplemental CAL is being issued to formalize your commitment to take specific actions to address these deficiencies.
Pursuant to discussions on April 14,1997, between you and Mr. Melvyn Leach, it is our understanding that you are currently pursuing performance improvements and have taken or will take the following actions prior to restarting LaSalle County Station-(1)
Develop a restart plan for LaSalle County Station to address corrective actions for -
the various performance deficiencies referenced above and transmit that plan to the NRC for formal public docketing. In developing this plan, you will also establish quantitative and qualitative criteria for assessing and monitoring the effectiveness of your corrective actions for each of the areas identified in the restart plan. The extent to which any action w!ll not be completed prior to restart will be identified and assessed to determine li rastart is acceptable with action pending.

You will keep the NRC updated on the projected startup date for the lead unit in order to facilitate advance planning of an NRC special inspection to be conducted prior to startup.. This NRC inspection will evaluate the operational readiness of LaSalle County Station and your staff.
(3)
You will meet with the NRC staff on approximately a monthly basis to discuss the results of the startup plan implementation and the extent to which the evaluation criteria for corrective action effectiveness have been met. These meetings will t,e open to public observation.
(4)
You will meet with the NRC once you have completed your high intensity training program for licensed operators to discuss both the results of this training initiative and the process and indicators you used to evaluate the ability of your licensed operators to start up and operate the units at LaSalle County Station. This meeting will be open to public observation.
(5)
You will meet with the NRC once you have concluded that one of the units at LaSalle County Station is ready for restart to discuss the basis for your conclusion.

Nothing in this confirmatory action letter is intended to preclude you from taking actions I
which you deem necessary to place the plant in a safe condition or maintain it in a safe condition in the event of changing conditions.
Pursuant to Section 182 of the Atomic Energy Act,42 U.S.C. 2232, you are required to:
1)
Notify me immediately in writing if your understanding differs from that set forth above; 2)
Notify me if for any reason you cannot complete the actions within the specified schedule and advise me in writing of your modified schedule in advance of the change; and 3)
Promptly notify me in writing if for any reason you cannot complete the actions discussed in items (1) through (5) above.
i Issuance of this Confirmatory Action Letter does not preclude issuance of an order formalizing the above comn.itments or requiring other actions on the part of the licensee; nor does it preclude the NHC from taking enforcement action for violations of NRC l
requirements that may have prompted the issuance of this letter. In addition, failure to take the actions addressed in this Confirmatory Action Letter may result in enforcement action.
