NRC INSPECTION REPORT 50-456/97002; 50-457/97002
Dear Mr. Stanley:
On February 21,1997, the NRC completed an inspection at your Braidwood 1 & 2 reactor
facilities. The enclosed report presents the resu'ts of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the
Braidwood facilities was generally characterized by safety-conscicus operations. We are
concerned, however, about a violation identified by NRC inspectors involving two
examples of failing to follow procedures by radiation protection technicians. Procedure
adherence problems that cross most of your organization have resulted in both cited and
non-cited violations were previously discussed in inspection Reports 96021,96019,
,

96014,96012,96011,96006, and 95017.
We are also particularly concerned about one violation involving inadequate corrective
actions. During this inspection period, inspectors identified the staging of materials on the
auxiliary building and fuel handling building roofs that were susceptible to being blown off.
It was conditions similar to these that caused the Unit 2 loss of offsite power and led to
the issuance of NRC violation 96002-01. Corrective actions to prevent recurrence
identified in LER 96-001 were clearly not implemented at the time of this roof inspection.
What makes this event more disturbing was the failure of station personnel to take
appropriate corrective actions once the inspectors identified the problem, and to provide
the proper control and oversight of contractor work. This event raises questions about the
ability of Braidwood Station to adequately imptoment corrective actions and to maintain
facility operation in accordance with those coo octive actions.
These violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that
,
you are required to respond to this letter and should follow the instructions specified in the
enclosed N >tice when preparing your response. Your response should not only detail the
actions to be taken for the specific issues identified but should also be broadly focused on
what actions you plan or have taken to ensure that your corrective actions process is
effective. The NRC will use your response, in part, to determine whether further
enforcement action is necessary to ensure compliance with regulatory requirements.
