Docket No. 99901031/85-01 NOTICE OF NONCONFORMANCE The NRC conducted an inspection on October 7-9, 1985, at your facility in Murrysville, Pennsylvania which was part of the NRC's review of outage activities associated with Omaha Public Power District's Ft. Calhoun facility.

During this inspection a review of Westinghouse's quality assurance and engineering activities related to the safety-related 4160/480 volt transformers for Ft. Calhoun was performed. Based on the results of this inspection, it appears that certain of your activities were not conducted in acccrdance with NRC requirements. These items are stated below.

1. Criterion III of Appendix B to 10 CFR Part 50 requires, in part, that measures be established for the selection and review for suitability of application of materials and parts that are essential to the safety-related functions of structures, systems and components.

Westinghouse Document No. OTT-1, " Dry Type Transformers Comercial Dedication Process", Part II, Section D.1 " Design Control" requires that materials used in the transformer being dedicated, including non-metallic materials, be compared to those used in the generically qualified transformer.

Contrary to the above, Westinghouse failed to document the selection and the review for suitablilty of application of materials and parts of the 4160/480 volt transformers for Ft. Calhoun that were determined to be essential to the safety-related functions of the transformers. (85-01-01)

2. Criterion III of Appendix B to 10 CFR Part 50 requires, in part, that measures be established for the identification and control of design interfaces and for coordination among participating design organizations.

Contrary to the above, Westinghouse failed to establish measures to control design interfaces and coordination among participating organizations which determined those components of the Ft. Calhoun transformers to be classified as critical to the safety-related operation of the transformers.

3. Criterion VII of Appendix B to 10 CFR Part 50 requires that measures be established to assure that purchased material, equipment, and services, whether purchased directly or through contractors and subcontractors, conform to the procurement documents. These reasures shall include provisions, as appropriate, for source evaluation and
selection, objective evidence of quality furnished by the contractor or subcontractor, inspection at the contractor or subcontractor source, and examination of products upon delivery.

Westinghouse Document No. DTT-1, " Dry Type Transformers Commercial Dedication Process", Part II, Section D.3, " Design Control" requires that information received from the vendor or customer on field experience to provide information on the transformers' design, will be retained.

Contrary to the above, Westinghouse failed to document objective evidence of quality furnished by the manufacturer on the 4160/480 volt transformers which were being procured by Omaha Public Power District for the Ft. Calhoun station. This evidence includes source evaluation or information from the manufacturer or other customers of the manufacturer on field experience. (85-01-03)
